Objective:To detect the expressing of TOPO-1 and CES-2 in metastatic colorectal cancer (mCRC) and to analyze their relationship with irinotecan chemotherapeutic, to explore the possibility of being to a factor as a predictor of irinotecan chemotherapeutic.Methods:Collected the patients who use CPT-11/5-FU/leucovirin(FOLFIRI) first-line chemotherapy in metastatic colorectal cancer between Jan.1,2014 and Dec.31,2014 at the Fourth Affiliated Hospital of Guangxi Medical University. Using the immunohistochemical(IHC) method detected the expression of human CES-2 and TOPO-1. According to the expression, taking them into high expression and low expression group respectively, followed up PFS, analysis of the expression of their relevance to the clinical characteristics, chemotherapy effect and survival time.Results:98 cases were collected. All cases were evaluable for the short-term curative effect and progression-free time (PFS), the overall survival (OS) need to continue to follow-up. Among,62 cases aged≤60,36 cases>60,51 cases is male,47 cases is female,49 cases of colon cancer,49 cases of rectal cancer,65 cases of tumor differentiation in high-middle,33 cases in low,60 cases is liver metastasis,38 cases is other transfer. In the all patients,46(46.9%) showed TOPO-1 high expression,52 showed TOPO-1 low expression, 53(54.1%) showed CES-2 high expression,52 showed CES-2 low expression. 98 short-term curative effect evaluable cases, respectively, CR 4 cases (4.1%), PR 35 cases (35.7%), SD 33 cases (33.7%), PD 26 cases (26.5%), RR (efficiency) 39.8%. The response rate of TOPO-1 high expression was 52.2% (24/46), the response rate of TOPO-1 low expression was 28.8%(15/52). The response rate of CES-2 high expression was 49.1%(26/53), the response rate of CES-2 low expression was 28.9%(13/45).Conclusion:It is no obvious correlation of the expression of TOPO-1, CES-2 with clinical pathological characteristics. It is not relevant between the expression of two indicator. The expression of TOPO-1, CES-2 in mCRC has significant correlation with the Irinotecan chemotherapy, short-term curative effect. TOPO-1 and CES-2 expression may can predict the Irinotecan chemotherapy of mCRC. They should be used as two predictors of curative effect of Irinotecan chemotherapy. TOPO-1 high expression, CES-2 high expression group of the patients than their low expression group patients have longer PFS, especially both TOPO-1 and CES-2 are high expression group. It may can serve as a predictor of prognosis in mCRC.
