Objective:To discuss the ultrasonic diagnosis of fetal single umbilical artery (SUA), the relationship between SUA and combined fetal structural abnormality, the relationship between SUA and chromosome abnormality and the effect of SUA on fetal growth and development.Methods:To analysis retrospectivly 90 cases with fetal SUA found by fetal prenatal ultrasound screening from January 2012 to September 2015 in the first affiliated hospital of guangxi medical university and collected their complete clinical datas and followed-up their pregnancy outcomes(including 48 cases with isolated single umbilical artery and 42 cases combined fetal structural abnormality).45 cases out of all pregnant womens with fetal SUA have acceptted amniotic cavity puncture or umbilical cord blood centesis for fetal karyotype analysis. Normal control group was 102 cases with double umbilical artery who were born in our hospital and were normal by prenatal ultrasound screening and followed-up observation after they were born.Results:1. SUA often combined fetal structural abnormality, and those structural abnormalities often were multiple. The top three types in all Combined fetal structural abnormalities respectively were cardiovascular system abnormality, nervous system abnormality and bone and body abnormalities; incidence of ventricular septal defect was the highest in Various kinds of cardiovascular system abnormalities.2. The difference of fetal structural abnormality’s incidence between the group with pregnant women’s age≥35 years old and the group with pregnant women’s age< 35 years old was no statistical significant (P> 0.05).3. Incidences of increased fetal umbilical artery S/D value, FGR and SGA were no significant difference between the isolated SUA group and the normal control group (P> 0.05).4. Average fetal birth gestational age and average birth weight of the isolated SUA group were lower than the normal control group (P<0.05); the pregnant women’s average age and average weight between the isolated SUA group and the normal control group had no significant difference (P> 0.05).5.The incidence of fetal chromosomal abnormality of the combined structure abnormality group was obviously higher than that of the isolated SUA group (P< 0.05); the incidence of chromosomal abnormality when combined multiple structure abnormalities was higher than when combined single structure abnormal (P< 0.05).Conclusion:1.Prenatal ultrasound screening could clearly diagnosis the fetal SUA.2. There was some correlation between SUA and fetal structural abnormality;There was no obvious correlation between SUA combined fetal structural abnormality and the age of pregnant women.3. Isolated SUA could cause certain effect on fetal growth and development.4.There was some correlation between SUA combined fetal structural abnormality and fetal chromosomal abnormality, and that chromosomal abnormality’s incidence of ocombined multiple structure abnormalities was higher than combined single structural abnormality fetus; one of the most common chromosomal abnormality of SUA fetus was three -body syndrome; 5.The prognosis of combined fetal structure abnormal was poor and the mortality in perinatal period was high,while the clinical prognosis of isolated SUA was good.
