ObjectiveThis study will discuss the possible acupuncture analgesia mechanism according to the comparison of clinical effect of Temporomandibular disorders dysfunctional pain, which have the efficiency of DNIC, by electroacupuncture at homotopic and heterotopic nerve segments. They may provides acupuncture clinical guidance and objective basis.MethodsEthical approval was obtained from the Guangdong province hospital’s ethical committee(NO:B2015-056-01), and was signed in Chinese Clinical Trial Registry(NO:ChiCTR-IOR-15006884).24 patiens (age range 18-60 years old) with Temporomandibular disorders dysfunctional pain participated in the study, they were all recruited from the department of Guangdong TCM hospital.With randomized controlled clinical method, subjects were assigned to one of four active groups:electroacupuncture applied at the face of patients (ST6-Jiache and ST7-Xiaguan acupoints) were administered to 2 groups of patients with Temporomandibular disorders dysfunctional pain, one group was administered with noxious stimulation and one with innocuous stimulation. While group 3 and 4 received the same means applied at the legs (the same segmental, ST36-Zhusanli and GB34-Yanglingquan acupoints), respectively. The whole 10 times conversations will meet 2-3times per week, each will lasts 30min. The outcome variables were the VAS and pressure pain threshold(PPT) measured before and after each treatments, and Multidimensional Pain Inventory(MPI) scores recorded at per-treatment、the third、the sisxth and post-treatment.The McGill scores will evaluate the subjects’general heath status at the first meeting and after the whole conversation. Data were analysed using the SPSS for Windows.ResultsThe baseline was confirmed by a one-way analysis of Fisher and Rank Sum Test for pre-treatment showing no significant differences.Both groups 1 and 2 shows significantly improvement in the pain symptoms according to the assessing of VAS(P<0.05)、MPI (P<0.05) and McGill (P<0.05)、 the changing rate of PPT (P<0.05). And differences were found between groups.Conclusion1、Electroacupuncture therapy for Temporomandibular disorders pain dysfunctional Syndrome has clinical curative effect.2、Noxious and innocuous stimulation with electroacupuncture on acupoints located in homotopic both can alleviate pain syndrome effectively, but didn’t tell the difference between each other. Heterotopic nerve segments didn’t showed the alleviation of pain effectively in this case, which possibly blame to the efficiency of DNIC. Accordingly, we suggested to choose the acupoints located in homotopic in the patients of TMD dysfunction pain.
