| Objective:To explore the effects of ultrasound on weaning for patients with mechanical ventilation using lung ultrasound and diaphragm movement scale, and to study the influence of TCM syndrome on weaning.Methods:Ultrasound is used to evaluate the pulmonary ventilation and diaphragm movement, and a new scale is established to evaluate the pulmonary ventilation and diaphragmatic movement named ultrasound score in this study. A total of 50 patients admitted in ICU in Xuzhou Central Hospital from Feb,2015 to Mar,2016 are included into the study; All Patients were scored using the the ultrasound scale. According to the weaning results, patients are divided into two groups. The success group with 28 patients and the failure group with 22 patients. ROC curve was used to determine the ultrasound score in weaning predicting.Meanwhile the influence of TCM syndrome type on weaning was analyzed.Results:The scores between the the successful group and failure group are different(P< 0.05). The area under the ROC curve is 0.683, and the score to predict the possibility of successful weaning is 8, the sensitivity is 54.5%, the specificity is 75%. The order of different TCM syndrome in the successful group from high to low are syndrome of turbid phlegm obstructing lung syndrome;phlegm and blood stasis obstructed in lung syndrome;phlegm heat in the lung syndrome;mental unconscious caused by phlegm syndrome;lung, spleen and kidney deficiency syndrome and vital qi deficiency and collapse with dyspnea syndrome, while in the failure group are, espectively is vital qi deficiency and collapse with dyspnea syndrome;lung, spleen and kidney deficiency; mental unconscious caused by phlegm syndrome; phlegm heat in the lung syndrome;syndrome of turbid phlegm obstructing lung and phlegm and blood stasis obstructing the lung syndrome.Conclusion:Ultrasound score can be used to predict the outcome of weaning and is effective; when the score is below 8 points, weaning will succeed with a great possibility; Patients with mechanical ventilation caused by TCM Syndrome of turbid phlegm obstructing lung have the highest success rate of weaning while vital qi deficiency and collapse with dyspnea have the lowest success rate. |
