The some researches shows Cerebral small vessel disease is the main cause of vascular cognitive impairment and age-related cognitive function decreased. Comparing with other diseases, Cer- ebral small vessel disease onset more hidden, progress more slowly and clinical symptoms fre-quently atypical. So cerebral small vessel disease is easy to be ignored. But cerebral small vessel disease once develops into to vascular dementia would seriously affecting the quality of life of patients and bring enormous economic pressure to family and social. Now the drugs can not effectively reverse the tendency that eventual develop into vascular dementia, but can delay the development process if early intervention treatment according with its risk factors. Combining with previous researches show Huayu tongluo mixture has the effect of "Bleeding blood stasis, new blood generation to make collaterals expedite", that can promote angiogenesis after cerebral infarction, ameliorate cerebral blood circulation and neurological deficits and improve cerebral small vessel disease caused cognitive dysfunction that by reducing the number of white matter degeneration, lacunar lesions and micro brain hemorrhage. The topic employ functional magnetic resonance imaging to observe the influence of Huayu tongluo mixture on various functional areas of the brain to further Discussion the probable mechanism of Huayu tongluo mixture that effect of improving cognitive function.MethodsAccording to inclusion and exclusion criteria,we make enrolled 40 patients and 15 cases of healthy volunteers as a blank group.40 patients were randomly divided into treatment group and control group, which 20 patients in each group.In addition to receiving regular treatment group based drug treatment and rehabilitation treatment, give oral the complexing Huayu tongluo mixture 1 bottle/day for 1 month intervention, The control group only received conventional medical treatment and rehabilitation, healthy blank group without any treatment. Treatment group and control group perform related cognitive brain function evaluation and overall cognitive function using Montreal Cognitive Assessment Form (MoCA) before treatment, and carry on functional magnetic resonance imaging (fMRI)before and after treatment for 30 days. we use brain function data auxiliary processing software (DPARSF)and statistical parametric mapping software(SPM8) make Posterior cingulate area that we select interested data as Preprocessing. And we make One-sample and two-sample t-test by using REST software to analysis. Observation Huayu tongluo mixture on the brain after treatment resting state functional connectivity patterns, and further explore the possible mechanism of improving cognition.Results1ã€The control group compared with blank group, after treatment we finded the connection of the right cingulate gyrus with left middle frontal gyrus /temporal gyrus/orbital frontal gyrus/right inferior frontal gyrus become strength,at the same time the connection with right middle frontal gyrus, Parahippocampal gyrus become wane and the feature connection of the left the cingulate gyrus with left temporal lobein(Superior temporal gyrus /Superior temporal).Cerebellum in control group patients.2ã€The Treatment group compared with blank group, after treatment we finded the feature connection of The right Posterior cingulate with The left inferior parietal lobule,The right side of the pillow gyrus/Hippocampus/Inferior frontal gyrus become strength and the feature connection with the right parahippocampal gyrus/orbital forehead gyrus/medial frontal upper gyrus/amount gyrus become wane.The feature connection of left posterior cingulate with right cerebellar/inferior temporal gyrus/superior temporal gyrus,the left inferior temporal gyrus/hippocampus/temporal gyrus become strength,and the connection with the left inferior parietal lobule/the inside inferior frontal gyrus/dorsolateral inferior frontal gyrus become wane.3ã€The Treatment group compared withontrol group, Huayu tongluo mixture intervention of MoCA score significantly higher after 1 month, prove that Huayu tongluo mixture having efficacy to improve cognition.ConclusionThe mechanism of Huayu tongluo mixture improve Cerebral small vessel disease caused cognitive dysfunction is not clear.
