| Background and Objectives: Primary immune thrombocytopenia(p ITP) previously known as idiopathic thrombocytopenic purpura( ITP), is the most common acquired,organ-specific autoimmune haematological disorders of children.Now its pathogenesis is not completely clear yet, the unbalance of humoral immunity,cell immunity disorders,abnormal secretion of cytokines,apoptosis of platelet and genetic or environmental factors all may be contributed to ITP.B cell activating factor belonging to the TNF family(BAFF) can promote the proliferation of B cell and antibodies production,the activation of T lymphocytes and its immune response;the B cell tolerance can be damaged and lead to all kinds of autoimmunity diseases under the high level expression of BAFF. Studies have shown that BAFF is expressed in a high percentage of patients with c ITP in adults,moreover,the PBMC and spleen MC of BAFF m RNA expression level significantly higher than normal.However there were few reports about the research on the BAFF and ITP in children.In order to explore the role on the mechanism in patients with ITP about BAFF and further clarify the pathogenesis of ITP,we have carried on the correlation research to expression of BAFF and IL-12,IFN-γ,IL-4 in patients with ITP.Methods:By ELISA quantitative detection of the level of BAFF,IL-12,IFN-γ and IL-4in plasma with different experimental groups which including the newly diagnased group,the treatment group and the control group.Besides, we also have detected the expression of BAFF and BAFF m RNA which derived from PBMC by RQ-PCR.What,s more,to explore whether there was correlation between the BAFF level and kinds of clinical parameters such as the age, gender,the number of platelet counts and cytokines.Results: 1.The BAFF level of the newly diagnased group was 431.82± 174.79pg/ml,which significantly higher than the control group(236.74±68.31pg/ml) and the treatmentgroup(252.62±63.72pg/ml);The expression of BAFF and BAFF-R m RNA in PBMC was significantly above other groups(P < 0.05),We also found that there was a negative correlation between the BAFF level and platelet counts(r=-0.560,P<0.05), but others were not(P>0.05).2.The levels of IL-12 and IFN-γ in the newly diagnased group were higher,(32.86±4.93 VS 17.27±1.88pg/ml;116.70±7.30 VS 71.76±7.48pg/ml),but IL-4 was low(36.47±3.82 VS 52.23±4.85pg/ml), the level of IL-12 and IFN-γ was positive(r=0.874,P<0.05).3.Research has shown that BAFF m RNA and BAFF level/BAFF-R m RNA were significantly positive(r1=0.737,r2=0.880,P < 0.05); there was a positive correlation of expression among BAFF level/BAFF-R m RNA and IL-12(r1=0.830,r2=0.867,P <0.05),but to IL-4 was contrary(r1=-0.855,r2=-0.860,P<0.05).Conclusion:1.BAFF may involved in the pathogenesis of ITP in children.2.Th1 / Th2 cells and their cytokines were abnormal in ITP.3. BAFF may via the BAFF-R signaling pathways play an immportant role in the development of ITP in children. |
