Objective Investigate the causes of the revision of the total hip arthroplasty and analyze the clinical effects through the long-term follow-up.Methods Between December 2004 and April 2011, in our hospital 48 (49 total hip replacement) patients underwent surgery of the revision total hip replacement. Complete follow-up data were obtained in 40 cases,8 patients did not show up for follow up,4 cases died of other diseases. The basic information of patients:gender, age, work, cause of the first operation, reasons of the revision arthroplasty, prosthetic life, renovation of the operation time and bleeding volume. Through the analysis of hip joint replacement and the revision surgery of the long-term follow-up results were evaluated through the Harris and VAS scores.Results The postoperative 1 month (80.12±6.12),one year (87.27±6.18), the last follow-up Harris scores (82.70±6.56) increased significantly than the preoperative Harris score(31.57±10.35)through the mid-long term follow-up(P <0.05). The postoperative 1 month (2.68±0.97),one year (1.55±0.94),the last follow-up VAS scores (1.58±1.33) decreased significantly than the preoperative VAS score (7.90±1.03) through the mid-long term follow-up(P <0.05).But there is no statistical difference between one year (1.55±0.94) and the last follow-up (1.58±1.33) VAS scores (P=0.867). And through the single factor analysis (K-M), found that the life expectancy of prosthesis with cemented and non-cemented prosthesis have statistical significance (P< 0.05), but gender and occupation have no significant impact.Conclusion The aseptic loosening of the prosthesis is mainly the cause of the revision surgery. Other causes are the surrounding femoral head dislocation and infection. By the last follow up of the hip joint arthroplasty, the patients had good hip joint function and were satisfied with the operation. Hip pain has been effectively alleviated But the function of the hip joint decreased by the last follow up.
