The Early Wearning Value Of The Concentration Of Serum Cystatinc In Acute Heart Failure Accompanying Acute Kidney Injury

Objective:To explore the prognostic value of the Cys-C for AHF with acute kidney injury (AKI).Methodology:An Retrospective analysis of 125 in-patients with AHF in the First Affiliated Hospital of Guangxi Medical University from January 1,2009 to December 31,2015 was made. The concentration of serum Cys-C, Cr and BNP after AHF in 0 hours, the first day, the second day and the third day was measured. Spearman correlation coefficient was used to analyze the correlation of Cys-C and Brain Natriuetic Peptite (BNP), Wilcoxon rank sum test to compare the differences in Cys-C、Cr and BNP between AHF group (control group) and AKI group (case group) in four different time, P<0.05, differences have statistical significance. Nemenyi rank sum test was used to compare them of case group. P<0.05, differences have statistical significance. Results:There were not statistical differences in baseline date of control group and AKI grou (P>0.05). The Cys-C and Cr were positively correlated with BNP(P<0.05), and the correlation degree of Cys-C is higher than Cr (r-value of Cys-C were 0.537,0.635,0.744 respectively in the first day, the second day and the third day; while the r-value of Cr were 0.445,0.528,0.650 respectively in the first day, the second day and the third day). After the occurrence of AHF, the levels of Cys-c, Cr and BNP increased with the passage of time both in control group and case group. After the comparison, we found that P<0.05, proving that the differences had statistical meaning. In case group,After the first day, the second day and the third day of the onset of AHF, should the Cys-C be taken as the indicator to judge AKI, the AKI incidence were 25/51 (49.0%),42/51 (82.4%),51/51 (100%); While should the KDIGO be taken as the indicator to judge AKI, the AKI incidence were 13/51 (25.5%),28/51 (54.9%),51 /51 (100%), suggesting that in two day, Cys-C can find more patients with acute kidney injury than creatinine and Cys-C has certain value in the diagnosis of AHF complicated with AKI and the efficacy is better than Cr.Conclusion:Cys-C has a early prognostic value of AKI than Cr in patients with acute heart failure...
