| ObjectiveTwo kinds of fracture prediction tools of risk assessment (Frax and bone strength composite index) survey of elderly hip fracture probability of the comparative study, the author hopes to provide better prediction method, early prevention to brittle fractures caused by osteoporosis, improve the life quality of the elderly population.Methods(1)The retired cadres retired Genenral Hospital of PLA Guangzhou Military Area medical cadres and cadres of seven families, six families of hospitalized patients in 262 cases. Mean age 82.44±6.470 years old, among them male 238 (90.49%), average age is 83.13±5.740 years old, female 25 (9.5%), average age 75.84±9.026 years old. The subjects were included in the study according to the inclusion criteria, and the bone density of the patients was examined, And according to the correlation of all four examinations of TCM, it can be divided into the kidney-Yang deficiency, Liver-Kidney Yin deficiency, spleen-kidney-yang deficiency And Qi Stagnation Blood Stasis; Among them,49 cases were examined their pelvis. (2) The bone mineral density was collected, and the data of the fracture risk was calculated on the FRAX website. The femoral neck width (FNW) and the proximal shaft length (HAL) of the femoral neck were measured, and the composite index of the bone strength was calculated. (3) All the data using statistical software epidate 3.1 summary entry, and pairs of recorded enrolled on; using the statistical software spss19 for windows to the data for statistical analysis, each set of data the mean± standard deviation form (X±S), using independent samples t test of each group were compared and analyzed; linear regression method was used to compare the HF>3%and bone strength index, a= 0.05 level test, to P<0.05 have statistical significance.Results(1) 263 subjects, FRAX calculates the risk probability≥3% of hip fracture in 29 cases, parts of the main fracture risk probability≥20% patients are 0 case. (2) Hip fracture risk probability is greater than or equal to 3% and bone strength comprehensive index through the comparison of the linear regression analysis method, visible, along with the increase of CSI, hip fracture was decreased; with the increase of the BSI, hip fracture showed a downward trend; with the increase of the ISI, hip fracture was decreased. (3)In this study,214 cases were selected as the study object of TCM dialectical classification, including 129 cases of bone mass abnormality group including bone mass reduction and osteoporosis. Group of abnormal bone mass include spleen-kidney-yang deficiency (37.79%)>Liver-Kidney Yin deficiency (33.90%)>kidney-Yang deficiency (22.18%)>Qi Stagnation Blood Stasis (6.10%).Conclusion(1) In the elderly in the Guangzhou area, the risk of hip fracture risk in patients with>3% was significantly higher than that of the major site of fracture risk in patients with>10%. (2) In this study, we must early intervention in the treatment of patients with osteoporosis in patients with less, and Guangzhou area of the old people’s living habits are closely related. (3) The comparison of FRAX and bone strength index (CSI, BSI, ISI) showed that the higher the baseline value of each comprehensive strength index, the lower the risk of hip fracture. (4) TCM syndrome type has great relationship with osteoporosis, in these people, the kidney-Yang deficiency and liver and Liver-Kidney Yin deficiency are higher and prone for osteoporosis. These two types of syndrome in clinical should pay more attention to early intervention treatment, in case of fragility fracture. |
