| Objectiveto study the changes of refractions and pseudo-accommodations of patients with high myopia and cataract after the treatments of phacoemulsification combined with intraocular lens implantation (TCL).Methodscollected the information of the patients who received the surgery of cataract phacoemulsification combined with intraocular lens implantation from May,2014 to January,2015 in our department ultrasonic emulsification by prospective randomized controlled study, among which the high myopic eyes were 30 (≥10D and axial length ≥27 mm), and non-myopic eyes were 25. Sla tape sighting mark was used to compare the changes of accommodation powers before and after operation of two groups. IOL-MASTER was used to measure the preoperative and postoperative corneal curvatures of high myopia group, ultrasound A and ultrasound B were used jointly to measure ACD, axial length (AL) and compare the changes of all parameters before and after operation. Optometry was carried out in different periods after surgery to compare the differences between postoperative refractive diopter and predictive refraction diopter before surgery.Resultsthe predictive spherical power of patient with high myopia before surgery was-3.81 ± 0.52D, while the average spherical power became -3.25±0.49D and-3.24±0.61D respectively one month and six months after surgery, the preoperative and postoperative comparative difference had statistical significance (P=0.000); the average preoperative ACD was 3.17±0.21mm, and changed to be 3.41±0.35mm and 3.42±0.29mm one month and six months after surgery, the preoperative and postoperative difference of ACD had statistical significance; the preoperative and postoperative differences of corneal curvatures had no statistical significance (P=0.62); the preoperative and postoperative comparative differences of axial length had no statistical significance (P=0.87); the average preoperative accommodation powers of patients with high myopia and non-high myopia were (3.17±0.91,2.83±0.97) D respectively, while the accommodation powers of two groups one month after surgery was 1.31±0.6 and 1.33 ±0.71; and (1.31±0.6 and 1.33±0.7)D six months after surgery. The preoperative comparisons and postoperative comparative differences of two groups had no statistical significances (P>0.05).Conclusionsspherical power of patients with high myopia had the tendency of hyperopia after surgery, nodal displacement caused by increased ACD after surgery may be one of the main reasons of hyperopia; the amplitude of accommodation of patients with high myopia and non-high myopia before and after surgeries had no differences. |
