Objective: To investigate the investigation of Karamay field operation of oil workers occupational stress level, effort reward imbalance by case-control method of exploration and hypertension related GR, β2-AR between genes and gene, gene and environment interaction. To improve the study of hypertension related to oil workers in Karamay field work, and to provide evidence for the prevention and treatment of hypertension. Methods: 1. This study adopt cluster sampling Karamay in the post 18 is less than or equal to age<60 years old, 2116 seniority is equal to or more than 1 years of oil workers(oil, downhole operation, heat injection operation) as the object of study by occupational stress inventory table occupational stress inventory Revised Edition(OSI-R) and effort reward imbalance(Effort-Reward has been reviewed questionnaire, ERI) survey was carried out. 2. A case-control study, from hypertension and non- hypertension population sample of 179 people(1:1 matched with sex, age and nationality) as the case group and control gro up, using polymerase-chain reaction-restriction fragment length polymorphism(PCR-RFLP) detection of GR, β2- AR gene polymorphism. Results: 1. Questionnaires were issued 2300 questionnaires, 2116 questionnaires were recovered, and the recovery rate was 92%. 2. Comparison of the survey of oil workers occupation tension with the national norm, the score is higher than the national norm; different gender in different occupation tasks and individual stress response score(P<0.05), male scored higher than female; different nationalities in different individual stress response score(P<0.05), higher than that of Han Minority; different cultural level of workers in different occupation task stress and individual coping resources score(P<0.05), bachelor degree or above than college and below high; different types of occupation tension in occupation task tension, the stress response of the individual scores of different(P<0.05), downhole operation scores were higher than those of oil and thermal injection operation, and operation of heat injection the oil workers scored higher than the scores of diffe rent seniority in occupation; task tension, personal resources score(P<0.05), different age less than 10 years scored higher than 10~20 years group, Length>20 years group, age>20 group scored higher than 10~20 years group and between different titles of occupational stress scores in the dimensions of different(P<0.05); Different gender, type of work, seniority in the low, medium and high tension group had statistical significance(P<0.05). 3. Different cultural degree in the extrinsic effort, significant differences in returns to work(P<0.05); different types of pay, internal input scores in different external(P<0.05), the highest score of downhole operation the oil workers, scored higher than thermal injection operation; different seniority pay, work reward score in different external(P<0.05), the group of >20 years of service the highest score; different titles, working on the score of different pay return on the outside(P<0.05), with higher professional title, each module of higher scores, suggesting that high pay- Title The return is not balanced; different marital status, job reward score different pay in the external(P<0.05), married scored higher than the unmarried; different economic conditions in the external and internal investment pay The difference was statistically significant(P<0.05), more than 3000 yuan of workers have higher scores than wages less than 3000 yuan. 4. Low, medium and high tension group in ERI is more than or equal to 1, ERI<1 Distribution of different(P<0.05); external effort in addition to individual coping resources, self health care, social support, rational response, and the rest of the positively related(P<0.05), returns from work and stress on occupational role, individual tight Zhang reaction, coping resources and sub Xiang Chengzheng related(P<0.05), internal input, in addition to entertainment, leisure, self health care, correlated positively with the rest of the(P<0.05); different culture degree, type of work, seniority, job title ERI is more than or equal to 1, ERI<1 differences in have statistical significance(P<0.05); 213 people with high blood pressure and 1903 people without hypertension. Gender, type of work, length of service, education level, professional title in the high blood pressure, there was statist ical significance in non hypertension(P<0.05);in the multiple logistic regression analysis on risk factors of hypertension, age, educational level, task, sense of responsibility, BMI, diet and smoking were the risk factors of hypertension, and the positive correlation between hypertension and hypertension. 5. The case group and the control group in occupational role, personal stress, coping resources and sub item scores of different(P<0.05), the case group scored higher than control group; case group and the control group in the low, middle and high level of tension distribution is different(P<0.05); case group and the control group in ERI is more than or equal to 1, ERI<1 Distribution of different(P<0.05). 6. GR BC L I and β2-AR 46A/G, β2-AR Gln27 Glu in case group and control group on distribution difference has statistical significance(P<0.05);(OR=0.454, 95% CI:0.206~0.995) increase in the risk adjusted BMI, smoking, passive smoking, drinking, taste, occupational stress level, pay and return on grading and GR BC L I site in the C G and GG suffering from high blood pressure, the β2-AR adrenergic receptor sites 46A/G Ag+GG suffering from hypertension risk increase(OR=0.332, 95% CI:0.150~0.735). 7. Using GMDR software analysis, the interaction between Bcl GR I andβ2-AR 46A/G was statistically significant. The interaction between occupational stress and Bcl GR I was statistically significant. Conclusion:1. O il workers occupational stress score is higher in males than in females and minorities is higher than that of the Han nationality, qualifications, seniority, professional title, income high workers nervous higher scores, scores of downhole operation phase was higher than production and thermal injection operation, and thermal injection operation score higher than scores of oil workers, unmarried is higher than that of the married. 2. Downhole operations pay- the most prominent return imbalance, with the length of service, job title, income increase, pay- return imbalance. 3. Gender, educational level, high level of tension, BMI, smoking, alcohol drinking, eating and drinking, emphasis on the risk factors of hypertension. 4. Bcl GR I site CG+GG genotype, β2-AR 46A/G AG+GG genotype is the susceptible genotype of hypertension. Interaction between 5. Bcl GR I and β2-AR 46A/G, the interaction between occupational stress and Bcl GR I was the combination of gene gene and gene environment interaction.
