The Application Of ERCP In Benign Biliary Stricture After Biliary Surgery

Objective: To evaluate the effectiveness and safety of endoscopy therapy for postoperative benign biliary stricture.Methods: Select 84 cases of patient with postoperative benign biliary stricture,52 cases had been diagnosed Sphincter Of Oddi dysfunction(SOD),they had been treated by endoscopic sphincterotomy(EST).Another32 benign biliary stricture patients had been treated by endoscopic retrograde biliary drainage.Observed clinical symptoms and liver function index changes after endoscopy therapy.Follow-up complication and efficay of endoscopy therapy.Results: The success rate of EST for patients with SOD is 100%. The response rate of stomach ache,nausea or vomiting,fever,jaundice,jaundice back pain is 65.31%、90.00%、87.50%、62.5%、80.00%.10 patients had serum amylase,1 patient had mild pancreatitis, 1 patient had severe pancreatitis, 1 pantient had cholangitis.Among the follow-up, there are 3 cases with cholangitis, 2cases with abnormal liver function and 2cases stricture resend.The curative rate is 95.65%. The success rate of stent placement is 100%. The response rate of stomach ache,nausea or vomiting, fever, jaundice,jaundice back pain is 54.17%、80.00%、88.89%、66.67%、100%.Preoperative and postoperative DBIL、TBIL、ALT、AST、GGT、ALP are statistically significant difference. 5 patients had serum amylase,4 patient had mild pancreatitis, 5 pantient had cholangitis.Endoscopy therapy did not effect on 1 case which had been treat by Roux-en-Y. 2 cases suffer bile duct stones and cholangitis.The curative rate is 88.89%.Conclusion: Endoscopy therapy for postoperative benign biliary stricture is effective and safe.
