Objective: The study was conducted to compare clinic staging of cervical cancer with operative pathological staging,further to explore risk factors for lymph node metastasis.Methods:The clinicopathologic parameters in 181 inpatients with FIGO(Federation International of Gynecology and Obstetrics)stage IA-â…¡B cervical cancer in the First Affiliated Hospital of Xinjiang Medical University from January 2010 to January 2014 were retrospectively analyzed.The risk factors for lymph node metastasis were evaluated by the way of univariate X~2 statsistic analysis or binary Logistic regression analysis.Results:The coincident rate of FIGO stages IA, IB, â…¡A, â…¡B with surgical stage were 57.9%(11/19), 73.6%(56/76), 52.1%(25/48), 39.5%(15/38) respectively.The overall accuracy rate of the clinical staging was 59.1%(107/181). IA, IB, â…¡A, â…¡B periods of lymph node metastasis rates were 10.5%(2/19),13.1%(10/76), 27.1%(13/48),50.0%(19/38). Univariate analysis showed that the serum level of squamous cell carcinoma antigen( SCCA)(X~2 =19.972 P<0.001), clinical stage( X~2 =20.930 P<0.001), tumor size(X~2 =5.722 P=0.017), neoadjuvant chemotherapy( X~2 =4.150 P=0.042), deep stromal invasion(X~2 =20.220 P<0.001), vascular invasion( X~2 =14.320 P<0.001), uterine tissue invasion(X~2 =7.549 P=0.006)were associated with pelvic lymph node metastasis. Multivariate analysis showed that SCCA(P< 0.001,OR=7.806), FIGO stage(P= 0.010,OR= 1.859), deep myometrial invasion(P= 0.007,OR=4.379), vascular tumor thrombus(P< 0.001, OR=7.312) were the independent risk factor for lymph node metastasis.Neoadjuvant chemotherapy can reduce the metastatic lymph nodes(P= 0.010,OR=4.129).Conclusion: With the increasing FIGO stage, the lower rate of two stages in accuracy, the higher rate of lymph node metastasis. SCCA, FIGO stage, deep myometrial invasion, vascular tumor thrombus were the risk factors for lymph node metastasis in cervical cancer. Neoadjuvant chemotherapy were protective factor of cervical cancer lymph node metastasis.It can provide the basis for early stage cervical cancer patients individualized treatment plan.
