Aim:This research is to explore the function and the way of clinical pharmacist to interfere the hypertension, and to evaluate qualified rate, compliance and the conduct of life of hypertensive duration of hospital stay as well as post-discharge by clinic treatment of hypertension united with pharmaceutical service.Methods:(1) The hypertensive patients were collected from Qingdao Municipal Hospital (Group) during January of 2013 to 2014.(2) Adopting random number table divided cases into two groups:group one is pharmaceutical service group, the other is control. The patients of pharmaceutical service group were treated with clinic treatment of hypertension, beyond that the clinical pharmacist providing the pharmaceutical service to those patients. After post-discharge the follow-up visits were completed by doctor and clinical pharmacist. The control was accepted with clinic treatment only. After post-discharge the follow-up visits were completed by doctors alone. The point-in-time of follow-up visits was the sixth months and the twelfth months after post-discharge. The observation index includes the qualified rate of hypertension, the safety of drug application, the follow-up visits of the sixth months and the twelfth months, compliance and the readmission rate.Results:(1) Comparing with control, the total effective rate of antihypertensive efficacy of pharmaceutical service group was 96.55% which was apparently higher than control that was 92.24%. It had the statistical significance (P<0.01); The experimental group 1 length of hospital stay was 8.5±1.8 days, The medicine accounted was (62.60±14.73)%,The control group length of hospital stay was 10.3±1.5 days,The medicine accounted was (64.33±12.86)%,The length of hospital stay and the drug proportion were shorter than control, which had no statistical significance. The total hospitalization cost and pharmaceutical cost between two groups got close, which had no statistical significance (P >0.05). The adverse event of pharmaceutical service group was lower than the control, which had the statistical significance (P<0.01)(2) Comparing with control, the effective rate of antihypertensive efficacy of pharmaceutical service group was obviously (P< 0.01) during six months. The compliance of pharmaceutical service group was superior to the control (P< 0.01) during the sixth months and the twelfth months. The self-management of pharmaceutical service group preceded control, excepting gave up smoking and drinking which had the statistical significance (P<0.05). The readmission rate of pharmaceutical service group was inferior to the control during the sixth months and the twelfth months, which had the statistical significance (P<0.05)Conclusion:For the hypertensive patients, to carry out pharmaceutical service path management, provide normalizes pharmaceutical care and pharmaceutical service could obviously enhance the effective rate of antihypertension, reduce incidence rate of adverse event, improve the qualified rate of hypertension and compliance after post-discharge, and reduce the readmission rate.
