Background:To evaluate the effect of Hemoperfusion(HP) plus Continuous Hemofiltration in patients with Hyperlipidemic Acute Pancreatitis(HLAP).Methods:This research selects 29 cases of patients who are diagnosed with hyperlipidemic acute pancreatitis from November 2011 to December2015 in China-Japan Union Hospital of Jilin University, 29 patients with HLAP are randomly divided into two groups, 15 cases of the group are treated by HP + CVVH( treatment group) therapy, 14 cases of the other group are treated by normal CVVH(control group) therapy,(1) Compare the clinical data of two groups of patients when they are admitted in hospital, including gender, age, blood glucose, TG level, and the APACHE II score, RANSON score, CTSI score;(2) Record the level of TG, APACHE II score, blood amylase, blood lipase, urine amylase,blood lactic, CRP, blood calcium of two groups of patients before treatment and after treatment on the 1th, 3th, 7th days;(3) Record the abdominal pain relief time, average hospitalization days, using time of the CVVH filter after the 1th, 3th, 7th days’ treatment;(4) Record the control of the complications before and after treatment.Results:(1) The TG level and APACHE II score of the treatment group and control group both show an obvious decline after treatment,and after the1 th, 3th, 7th days’ treatment, the TG level of the treatment group show an obvious decline than the control group, the difference is statistically significant( P < 0. 05); The treatment group show an obvious decline in the APACHE II score than the control group on the 3th, 7th days’ treatment, the difference is statistically significant(P < 0. 05);(2) The blood amylase, blood lipase, urine amylase, blood lactic and CRP of the treatment group and control group both decrease obviously, but the level of blood calcium has no statistical difference(P>0.05). Except blood amylase, blood lactic, CRP on the 3th day and the CRP on the 7th day,the treatment group show an obvious decline than the control group(P<0.05), the decline tendency of the two groups have no statistical difference in other biomedical criterion(P>0.05);(3) The abdominal pain relief time and hospitalization days of treatment group are less than the control group, the difference is statistically significant(P < 0. 05),and the using time of the CVVH filter of treatment group is longer than the control group, difference has statistical significance( P < 0. 05);(4)The incidence of various complications of the two groups has no statistical difference(P>0. 05), but the treatment group is better than the control group in controlling infections and renal insufficiency,difference has statistical significance(P<0. 05).Conclusion:(1) Hemoperfusion plus Continuous Veno-Venous Hemofiltration can lower blood TG level effectively, and can improve the biomedical criterion effectively;(2) Hemoperfusion plus Continuous Veno-Venous Hemofiltration can extend the using time of filter, extend the treatment time of blood purification for once;(3) Hemoperfusion plus Continuous Veno-Venous Hemofiltration can relieve the clinical symptoms, shorten the hospitalization days. |
