Objective:To analyze and compare the advantages and disadvantages of 4commonly used palliative biliary drainage for the patients with malignant obstructivejaundice. To provide reference for treatment of malignant obstructivejaundice. Methods:We collected 74 cases of malignant obstructivejaundice, which were dignosed in Our hospital from May 2014 to May 2015.According to their imaging results before operation, none of them was suitable(or can not) for radical resection and some palliative treatments were performed separately.including cholecystojejunostom( n=14)laparoscopic choledochoduodenstom( n=18) and endoscopic metal biliary stent(n=21),endoscopic plastic stent(n=21).The parameters analyzed were the operative time, jaundice reducing effects, duration of postoperative hospital stay, cost performance, prevalence of early and late complications Results:Endoscopic metal biliary stent and endoscopic plastic stent group had shorter hospital stay and less operative time. Laparoscopic choledochoduodenstom had less early and late complications, while nosignificant differences were found in reducing jaundice and survival time. Conclusion:Both laparoscopic choledochoduodenstomy and endoscopic biliary stent are effective methods to relieve biliary obstructionis for malignant obstructivejaundice. Laparoscopic choledochoduodenstom had less early and late complications. With a significant shorter operative time, duration of postoperative hospital stay, and postoperative fasting time, endoscopic biliary stent can rival the cholecystojejunostomy and laparoscopic choledochoduodenstomy.
