| Objective:To explore the diagnostic value of endobronchial ultrasound-guided transbronchial needle aspiration(EBUS-TBNA) in small cell lung cancer.Methods:354 patients with thoracic masses and/or mediastinal-hilar lymphoadenopathy proved by CT scan who underwent EBUS-TBNA at the First Affiliatd Hospital Zhejiang University from January 2013 to May 2014 were included in this study,Analysis the diagnosis value in SCLC.Results:A total of 50 patients were selected, with 30 males and 20 females, and the average age was 49±17.0 years old (ranged from 15 to 83 years).43 patients had different degrees of fever. There was no difference between this two groups in gender, age, average hospitalization time, clinical manifestation, white blood cells, neutrophils ratio, C-reactive protein and urea nitrogen. The rate of complying with cryptococcal meningitis in immunocompromised group was higher than that in immunocompetent group. Ten patients died during hospital stay,13 patients’ condition deteriorated, while 27 patients got better. A total of 49 patients had a final diagnosis of SCLC. The overall diagnostic yield of EBUS-TBNA for SCLC was 93.9%(46/49). Of them,4 cases diagnosed by cytology and the positive rate of 8.2%; 30 cases diagnosed by histological HE staining, and the positive rate was 61.2%; The remaining 12 cases whose cytological and histopathological results were all negative were diagnosed by immunohistochemistry.Conclusion:EBUS-TBNA has a high diagnostic yield in SCLC. It can be recommended for patients suspected to have SCLC, even if other diagnostic tests have failed. |
