| Objective:Hand in human production and life plays an important role. With the progress in raising the level of mechanization and industrialization, increasing rate of occurrence of hand injury. At present, China trauma research are concentrated in areas with developed economy, high level of industrialization, due to differences in area and population, constitution and distribution of hand injury also exist differences. Access to relevant information, about northeast of hand trauma research is relatively small, there is no study of Siping area of hand trauma related reports. This research is for the purpose of through collected system and finishing admitted to a hospital, Siping City, Jilin Province, in January 2012 to December 2014 1100 cases of hand trauma hospitalized patients with clinical data, in order to understand the Siping area hand injuries occurred in the characteristics and rules, for the rational allocation of medical resources, reduce hand external injury, provide the basis for the occurrence of.Methods:All the cases from January 2012 in a hospital of Siping City in December 2014 during the hand injury patients, a total of 1100 cases. Using retrospective study method, according to the research content, design questionnaire, from the hospital electronic medical records system to extract the relevant information. Its content mainly includes:(1)The patient’s basic information: name, sex, age, occupation, marital status, education, years of admission, month of admission, date of admission, discharge date, length of hospital stay, hospital costs, mode of payment of medical treatment.(2)The patients with disease information: injury time and damage to the visiting time, damage side, at the site of injury, tissue damage, whether the site, type of injury, cause of injury.(3)The treatment of disease information: anesthesia, operation mode.Results:(1) 1100 cases of hand injuries in hospitalized patients, male patients in 862 cases, accounting for 78.36%, 238 cases of women, accounting for 21.64%. The proportion of male patients and female patients was 3.62:1.(2) The youngest only 1 years old, 89 years old, the average age was 38 years old, and the number of patients with the most of the age group was 38-47 years old, 298 cases, accounting for 27.09%, followed by 48-57 age group, 19.45% cases accounted for 214; 204 cases accounted for 18.55%; 58 years old and above 102 cases, 18 cases of hand trauma, 93 cases, only 8.45%.(3) The number of farmers in hand injury was the most, 587 cases accounted for 53.36%, followed by workers, 304 cases accounted for 27.64%, while the number of cases of hand trauma, 57 cases, accounting for 5.18% of the total.(4) The proportion of patients with middle school education level was the highest,witch was 52.18%,and the education level of primary school and the following 26.18%.(5) In this group, 1100 patients with hand injury, with open hand injury, accounted for 96.00% of the total number of cases, closed hand injury accounted for only 4.00%, and no significant difference between the years. There was no statistical significance between the injured side. This group of cases, wrist and nearly palm damage rate is on a downward trend year by year, the upper arm injury rate downward trend more obvious, In 1100 cases with upper arm injury in 23 cases, rate of 2.09%; 84 cases with injury of forearm, incidence of 7.64%; with wrist injury in 85 cases, incidence rate of 7.73%; hand injury with 160 cases, incidence of 14.55%; injury of finger containing up to 880 patients, rate of 80%. The injury site of hand injury was decreased by the distal to the proximal end of the injury.(6) Finger injury, with finger injuries most 400 cases(36.36%), followed by the index finger, 372 cases 33.82%, third place is ring finger, 329 cases accounted for 29.91%.(7) In 459 cases with multiple injuries, 641 cases were injured, and 58.27% cases with non multiple injury. The incidence rate of different damage types was different( =35.79 P<0.0001). In this group, there were 430 cases with multiple injuries, 168 cases with multiple injuries, 320 cases with multiple injuries, 174 cases with multiple injuries, and 54.38% cases with multiple injuries.(8) From the perspective of histology, the skin lesions were the highest in 1056 cases(96.00%), 761 cases(69.18%), 720 cases(65.45%) of bone joint injury, the three part was the top three.(9) Hand trauma patients in the injury time to 0 to 5 when patients with less, of which 2 were treated with at least 4 of patients; 6 after the gradual increase in the first 9 hours after the first injury gradually decreased, at 13, rose again, second when the 14 peak, then gradually down to the bottom. Every day the number of patients with the largest number of time to 8 to 17, the total proportion of 65.00%. In October, there were 14.55% cases, 160 cases, 120 cases, cases in May, the rest of the following months, 10.00% patients were treated with the least, 58 cases, accounting for 5.27%. Four quarters of the distribution of the cases showed a gradual upward trend. After 2-4 hours(excluding 4 hours), 650 patients were treated with 59.09% cases. Within 6 hours, the patient reached 95.36%.(10)The types of hand injuries were related to the gender,age and occupation of the patients(P<0.05).(11) 1100 cases of patients with hand injuries, the shortest hospitalization for 1 days, the longest 170 days, the average length of hospital stay was 11.37 days; hospitalization expenses of at least 266 yuan, up to 96944.00 yuan, the average cost of hospitalization for each case was 11130.61 yuan. Hospitalization expenses of 10000 yuan and more than 454 cases accounted for 41.27%. The total cost of hand injuries in hospital is increasing year by year, and the cost per person is increasing year by year.Conclusion:1.There were male and female, but male was more than female, the ratio of male to female was 3.62:1. The age groups were the highest, and the age group was the highest in 38-47 years old.2.Open injury in hand injury more than closed injury, left hand more than the right hand, fingers were injured, in which most of the injured patients. Multiple site damage is slightly less than the single site damage.3.Workers in the type of damage to the main cutting injury, and the farmers more than a wound. Damaged tissue is the most common skin, followed by tendons and bone joints.4.Injury time was more common in the daily at 9 and 14, in October and the fourth quarter; most patients can be treated at 2-4 hours after injury.5.The types of hand injuries were related to gender, age and occupation.6.The majority of the patients in hospital cost 10000 yuan, the cost of hospitalization per day is increasing year by year. |
