Objective:This study is to identify and quantify temporal and spatial parameters and joint kinematic patterns by three-dimensional gait analysis (3DGA) in hemiparetic cerebral palsy children, to provide reliable experimental basis for a better rehabilitation treatment.Methods:A total of 20 children with hemiparetic cerebral palsy and 10 normal children were selected. Three-dimensional gait analysis was used to get the temporal and spatial parameters and joint kinematic patterns of all the children.Results:1. The paretic side were characterized by significant shorter stance phase and longer swing phase, than non paretic side and healthy children (P< 0.05). And hemiparetic children walked with significantly reduced step length, stride length, velocity and cadence compared to healthy subjects (P<0.05). However longer step width increased was noted in hemiparetic children compared to healthy children (P<0.05).2. As for the joint kinematic patterns during stance phase, hemiparetics displayed higher values of hip flexion, smaller ankle dorsal flexion and higher values of ankle valgus in paretic side of hemiparetics and larger knee flexion and extension in non paretic side of hemiparetics than healthy subjects (P<0.05). There was no difference in ankle varus, internal and external rotation, knee valgus, varus, internal and external rotation and hip extension, abduction, adduction and internal rotation between hemiparetics and healthy subjects(P>0.05). Between hemiparetic children, the less ankle dorsal flexion and more plantar flexion abilities, less knee flexion and extension, smaller hip flexion and more interenal rotation were observed in paretic side than non paretic side(P< 0.05). There was no difference between paretic side and non paretic side in Ankle and knee valgus, varus, internal and external, hip extension, abduction, adduction and external rotation(P>0.05).Conclusion:The 3DGA could quantitative analysis hemiparetic gait, the hemiparetic cerebral palsy children generally present an asymmetric gait pattern due to the pathology and search of better stability to optimize gait. Stance phase abnormal gait may give priority to with abnormal ankle; 3DGA plays an important role in rehabilitation treatment strategy formulation.
