| Objective:According to observing the clinical characteristics and cardiovascular changes in KD patients 10 years before and after, further to explore the variation of clinical characteristics to early diagnosis KD and prevent coronary heart disease.Methods:Methods retrospective analysis of 600 cases during 2001-2003 period and 2040 cases during 2011-2013 period hospitalized in Children’s Hospital of Chongqing Medical University. Recording respectively the onset age, Male/Female ratio,onset time, main manifestations,ratio of iKD,results of CDFI examination,time and effectiveness of using IVIG. Correcting the results of CDFI examination 10 years before according to the current standard. All the data use SPSS17.0 for statistical analysis,with P<0.05 for the difference was statistically significant.Results:Compared with a decade ago, the incidence of male is higher, but there is no significant difference between the two groups(1.66:1 vs. 1.76:1, P=0.510).The onset age is younger (31.20±0.55m vs.33.09±0.89m, P<0.05),especially the incidence of KD in infants obviously improved (23.6% vs.10.8%, P<0.05).In morbidity season distribution, spring summer two seasons primarily,but they are winter spring in previous years. May became the new peak.The incidence of iKD is higher than before(27% vs.22.2%, P=0.017),especially the ratio of iKD<lys is increased,the difference has statistically significant(P=0.003). The recurrence rate of KD is higher than the decade year,but there is no statistically significant difference(1.4% vs. 0.5%, P=0.081).The most common manifestation is fever,the second is the change of lips and mouth, then is binocular conjunctival congestion,there is no obvious change in two groups.But the incidence of cervical lymph node change is decreased(57.9% vs.67.8%,P<0.05).The incidence of coronary artery lesion is slightly decreased,but there is no significant difference(57.7% vs.58.9%, P=0.593), mainly coronary artery dilation. The incidence and position distribution of CAA is not changed much, But incidence of the coronary thrombogenesis has a increasing trend. In the aspect of valvular regurgitation, the incidence of mitral valve regurgitation is higher(31.8% vs.25.2%, P=0.003),but the basic trend—tricuspid regurgitation primary, then mitral valve regurgitation and pulmonary regurgitation, Aortic Regurgitation rarely-remained consistent.The ratio of using IVIG in KD patients is improved(91.7% vs.88.3%, P=0.011). The incidence of IVIG intolerance is higher,but here is no statistically significant difference(6.5% vs.4.7%, P=0.126).Conclusion:The onset age is younger than the decades years,especially the incidence of KD in infant is obviously increased. The onset seasons became spring and summer, the peak in May. The incidence of iKD is higher than before,but the the incidence of coronary artery lesion has not obvious changed. |
