| objective:To evaluate the risk factors of ovarian function in young patients with early cervical cancer, and the risk factors for ovarian function preservation in young patients with early cervical cancer;Materials and methods:Computer retrieval of relevant databases, Pub Med, MEDLINE, EMBASE, CBM, CNKI, Wanfang, VIP, Chinese biomedical literature database, retrieval time in 1980.1- 2016.1, language restrictions for Chinese, English, the documents obtained first by reading abstract for preliminary screening, and then further by reading the paper quality evaluation, finally retained studies that met the inclusion criteria, and then the literatures data extraction,Rev Man5.0.stata12.0 software was used for statistical analysis, the observed heterogeneity, reason analysis, choose the suitable model, map corresponding to the forest, the funnel plot, when necessary for subgroup analysis and sensitivity analysis. Results:A total of 20 articles were included in the literature, including 16 in English and 4 in Chinese, Ovarian transposition was performed in all patients with cervical cancer. The ovarian function and the occurrence of ovarian complications were followed up. Most of the literature is a retrospective case study,A total of 733 patients were included in the study. According to the intervention measures, patients were divided into three groups, the operation group, the number of patients without radiotherapy and chemotherapy group was 402, 94 patients after the operation of brachytherapy(BR) group, 237 patients with postoperative external irradiation group(EBRTBR). Meta analysis showed that, for young patients with early cervical cancer, ovarian transposition in patients with simple surgery, or postoperative radiotherapy group, were able to retain ovarian function, The chances of simple operation group of ovarian function preservation for 88.5%[95%(80.1-97.8), postoperative close radiotherapy group ovarian function to retain the chance for 93.7%[95%(86.4-101.6)], the chances of postoperative external irradiation groups of ovarian function preservation for 58.4%[95%(50.2-68.0). for young patients with early cervical cancer, The main reason for the failure of ovarian function retention was related to the dose of radiotherapy after surgery, followed by the age of patients, the probability of ovarian function retention was significantly increased in patients under 40 years of age; In the brachytherapy(BR) group ovarian irradiation dose was low. There was no significant difference in the rates of ovarian function between the simple operation group and the postoperative brachytherapy group, OR=2.63,95%CI(0.32,21.79), P=0.37; Compared with the external irradiation group, the latter was significantly lower than the external irradiation group, the difference was statistically significant OR=9.95,95%CI(5.75,17.22), and P < 0.00001; The chances of ovaries displacement of postoperative complications, statistical analysis results 85.1%[95%(74.9.1-97.1)]. Conclusion:Ovarian transposition is an effective measure to preserve ovarian function in young patients with early cervical cancer, and it can avoid the damage of the ovary and improve the quality of life of patients, it is worthy of clinical application;... |
