Respiratory failure and cardiac damage is a common cause of death,as in recent years, progress and popularity of ventilator-assisted therapy, the relative proportion of heart damage leading to death by a gradual increase, the relative increase in the proportion gradually lead cardiac cause of death, as soon as possible It found that due to acute organophosphate poisoning induced myocardial injury, give specific protection to positive cardiac treatment, prevention of complications, improve the success rate is essential.But also can effectively shorten hospital stays, reduce the burden on the patient in many ways.Therefore fully aware of cardiac troponin I, ECG and other economically and effectively, to check the feasibility of high significance in the course of acute organophosphate poisoning may be a better choice effectively detect, to reflect the situation of cardiac injury, for given to prevent complications.Objective: To observe the basic information and related laboratory findings in patients with acute organophosphorus pesticide poisoning admission, its retrospective analysis,discusses acute organophosphate poisoning in early cardiac damage related indicators to monitor and assess whether the prognosis instructive. develop systematic, effective and comprehensive treatment plan is very important.Methods: January 2014- December 2015, Jilin University First Hospital emergency medicine all acute organophosphorus poisoning patients admitted to the hospital a total of120 cases.After discharge criteria to filter into the group of 82 cases of treatment group, 20 cases in our hospital for medical examination of healthy adults as a control group.Record the amount of patients treated with poison, poison to the hospital and poison time to time gastric lavage general admission information. Enzymes line at different times, troponin c Tn I,dynamic ECG monitoring, detecting more than the control group on admission 4h indicators that point in time. On data collected by statistical methods for analysis and processing,compared with the control group was statistically significant if it has clear whether it has significance in predicting poor prognosis.Results: 1, the treatment group and the control group at each time point 4h elevated serum enzyme concentration was statistically significant(P <0.05);2, the treatment group at each time point c Tn I concentration and the control group 4hc Tn I concentration increased with statistical significance(P <0.05);3, mild poisoning group c Tn I concentration in comparison with the control group c Tn I concentration: c Tn I was significantly higher than(P<0.05), while CK and CK-MB concentrations compared with the control group, P >0.05 difference was not Statistical significance.Severe poisoning group c Tn I concentration with the control group, compared with a statistically significant(P<0.01), the concentration of CK and CK-MB elevation than the control group also statistically significant(P<0.05), Moderate to severe poisoning of the indicators between the two groups compared with each other,showing moderate group c Tn I concentration of the heavier group of low degree, the difference was statistically significant(P<0.05),CK-MB concentration is also lower than the severe group, The difference was statistically significant(P<0.05), serum concentrations of CK was no significant difference(P> 0.05).4, Q-T interval prolongation share in all types of arrhythmias largest proportion, about 58.5%.Conclusion:(1) enzymes, cardiac troponin c Tn I can reflect the damage caused by AOPP, was positively correlated with the degree of intoxication, for the evaluation of prognosis significance.(2) compared with enzymes troponin c Tn I in the heart damage caused by AOPP, higher sensitivity, can use this as an indicator of prognosis and evaluation of clinical criteria injury.(3) QT prolongation in the largest share of various types of arrhythmia, ECG changes of acute organophosphate poisoning is the most common type QT prolongation, considering the poor prognosis have an indirect relationship can react to acertain extent of myocardial injury case, predicting prognosis also has a certain impact.
