Thesis Purpose:Take one provincial prison inmates as object and based on the medical records, this thesis is to do a review for PTB infection situation of prison inmates which can analyze the risk factors and the onset time features. It helps to provide the basis for effective prevention and control PTB and developing prison TB prevention and control measures.Methods:Collected 2240 cases of medical information in one provincial prison system the newly diagnosed PTB prison inmates from January,2003 to December,2013. In cases of the same period Non-TB in the diagnosis of other disease 5903 cases as control, using self-designed questionnaire, we do review through the cases, controls the clinic and hospital medical records that inputting and database established by Epidata 3.0 software, statistical analysis using SPSS17.0 software, and research factors and incidence of PTB relationship using single factor analysis and logistic regression analysis. We do multi-factor unconditional logistic regression analysis and the onset time relationship analysis according to variables that the single factor analysis was statistically significant and the literature on the basis of thought to be related.Results:1. Investigate cases 2240 cases, among which were 2168 male,72 female, men were more than women that men and women sex ratio is 30.1:1. The youngest was 16 years old, the biggest was 81 years old, and the average age was 30.96±9.24 years old. Control group 5903 cases, among which were 5674 male,229 female, men were more than women that men and women sex ratio is 24.8:1. The youngest was 18 years old, the biggest was 87 years old, and the average age was 37.74±11.98 years old. The earliest cases confirmed in January 12,2003 at the latest to December 31,2013, focused on between 2007 and 2011.2. Onset case occurred every year in one provincial prison system every unit. The total peaks concentrated during 2007 to 2011 that coincided with the time when one provincial prison system adopted the form of X-ray radiography for PTB census since 2007. The number that PTB found increased obviously from 2007-2009 and has declined each year gradually and smoothly since 2010.3. According to the results of logistic regressions, gender, age, occupation, study in the middle school or not, household areas, Diabetes Mellitus, the other basic diseases, drug, alcohol, marriage and BMI are all influencing factors. Among them, the relative risk for male is higher than female, for younger is higher than for older. From occupation, the RR for workers is the lowest, for not studied in the middle school is higher. The RR for household areas in the west and northeast region is higher, for Non-DM and non-the other basic diseases persons are higher, for persons who engaged in garment processing, metal processing and manual laborers after jailed is higher, for drug addicts is higher, for those who have no history of alcoholism is higher, for those who have no marriage is higher, for lean BMI is higher and obese BMI is lower.4. The onset time relationship analysis. Survival analysis showed that about 26% prison PTB patients were found before imprisonment 260 days, about 50% have been found before imprisonment 505 days,about 75% have been found before imprisonment 950 days.COX regression results can be seen, whether or not graduated from middle school, other basic disease, gender, age and BMI these risk factors associated with the onset time strongly. We can get to know that it’s more easily to be attack in a short time for not studied in the middle school, no basic diseases, male, young adults and thinner person.Conclusion:1. Paying attention to PTB in the inmates at higher risk. Male, young adults, junior high school diploma, household registration in west and northeast region, the inmates who engaged in garment processing or metal processing, lean BMI will be PTB high-risk groups that we should pay highly attention in PTB prevention and control work. It’s more easily to be attack in a short time for not studied in the middle school, no basic diseases, male, young adults and thinner persons who should get more concern.2. The key point for prison to control PTB is to find the infection source as soon as possible. Through this study, we get to know about 25% prison PTB patients were found before imprisonment 260 days, about 50% have been found before imprisonment 505 days,about 75% have been found before imprisonment 950 days.3. This census role for inmates PTB is positive. One provincial prison started to adopt the form of X-ray radiography for PTB census every two years since 2007 for all inmates. This study shows that the total peaks concentrated during 2007 to 2011 which increased obviously from 2007-2009 and has declined each year gradually and smoothly since 2010.
