156 Cases Of Neuroblastoma Clinical Retrospective Analysis

Objective:To investigate the clinical featuress, treatment and prognostic factors of patients with neuroblastoma.Methods:Clinical data of 156 patients with neuroblastoma with extensive follow-up,treated in our department between September 2008 and September 2014.Results: ①This study 156 cases were inclouded, the Male to female ratio is 1.2:1. the average age is 38.8 months, the median onset age is 4 months.Primary tumor in retroperitoneal and adrenal gland District 106 patients,mediastinum 42 cases, pelvic 6cases, faciomaxillary region 1 cases,neck 1 cases. INSS stage for I stage in 3 cases, Ⅱ stage in 11 cases,Ⅲ stage in 34 cases,IV stage in 102 cases,IVs stage in 6 cases.②65 cases received follow-up got all treatments, follow-up rate was 100%, the follow-up time was 8-68 months.To the end of the follow-up time was 2014.09.30,overall survival rates were 63.6%.1-、2-and 3-year survival rates were 96.8%,84.4%, and77.6%,survival rates covered Ⅰ、Ⅱ and Ⅳs stages were 86.4%,survival rates covered Ⅲ、Ⅳ stages were 46.48%.Among 34 cases of survival,22patients (70.6%) achieved complete response(CR),9 patients(26.5%)achieved partial response(PR).③Single factor analysis showed that prognostic factors inclouded whether complete resection of the tumor, amplification of MYCN,bone marrow metastasis and bone metastasis, serum NSE (>100mg/L).④Multivariate results indicated that the independent prognostic factors inclouded whether complete resection of the tumor, amplification of Gene Copies of MYCN, bone marrow metastasisConclusion:Neuroblastoma in children who had been advanced were too late,they shoud supported by the Individual comprehensive therapy included resection, concurrent radiotherapy and chemotherapy. Whether complete resection of the tumor, amplification of Gene Copies of MYCN, bone marrow metastasis and bone metastasis significantly affects the prognosis, total removal of the tumor prolongs the survival time of the herald. Diagnosis gene of MYCN, examinations of bone marrow and serum NSE have the important clinical significance for prognosis.
