preface:The research comes from a preliminary experiments of peptic ulcer efficacy. The experiment aimed to investigate the mucosal protective agent for peptic ulcer healing quality. In the pre-experiment, We found that there are significant differences in various stomach diseases of the collagen fibers expression of antral mucosal biopsy tissue. Tgf-β1 is one of the most important growth factors which affect the synthesis and decomposition. In a further review of the literature, We recognize that Tgf-β1 plays an important roal in the outcome of the intervention of the stomach diseases. Type IV collagen occupy a key position in tumor determing aggressive and metastasis which regulation by Tgf-β1. However, in previous studies, most of the literature for studies involving fiber expressed are in the kidney, lung and skin tissue. Research on gastric disease is not complete of collagen fibers expression and expression from. The system of interrelation between Tgf-β1 and expression of collagen of anteal mucosa in the disease progression is not complete. So is this study come.Diagnosis of current gastic disorders primarily through clinical symptoms, physical and chemical testing, imaging, biopsy and other means. Compared with conventional surgery, Endoscopic mucosal biopsy to obtain specimens were less invasive, convenient operation, less side effects, less pain, less pay above the accuracy. Even multiple biopsies is allow, if necessary. Through this study, endoscopic antral mucosal biopsy specimens were stddied. Through the analysis of the expression of different gastric antral mucosa tissues of Ⅰ, Ⅲ,Ⅳ collagen, to explore the correlation between the expression of tgf-betal and outcome and gastric mucosal disease, from the collagen different time morphological characteristics in different pathological tissues in proportion to explore the occurrence and development of gastric cancer. Discussion morphological features of gastric cancer development process of collagen deposition in different periods. Trying to find out the correlation between expression and antral mucosal diseases antral mucosa between collagen fibers, infer progress and prognosis of the disease.The first part:the collagen fibers of antral mucosal tissues compared differencesObjective:To compare the different periods of the antral mucosa in pathological specimens, the differential expression of collagen fibers.Materials and methods:a retrospective analysis from 2013 November to 2014, Marchin our hospital 50 cases of gastric antrum mucosa samples, according to the pathological results were divided into 6 cases of the normal control group,20 cases of gastric ulcer,13cases of precancerous lesion, adenocarcinoma of 11 cases. Male 28 cases, female 22 cases, normal control group which male 1 cases, female 5 cases,gastric ulcer group male 13 cases, female 7 cases, precancerous lesion group male 7 cases, female 6 cases, adenocarcinoma group male 7 cases, female 4 cases. The average age of the normal control group is 36.17+10.91 years, ulcer group was 47.70+13.94 years, precancerous lesion group is 47.08+7.44, adenocarcinomagroup is 65.55+8.36. The four groups of patients were collected data:(1) general information:gender, age, name; (2) the experimental examination:carbon thirteen breath test, rapid urease test; (3):endoscopic mucosal hyperemia and edema,erosion, ulcer, healing ulcer activity period, scar ulcer, mucous membrane, change,rough varioliformis uplift type gastric cancer, superficial gastric cancer, depression,gastric cancer, polyps or mushroom type gastric cancer, gastric ulcerated carcinoma,infiltrative gastric carcinoma; (4) histopathology HE staining results:inflammation,granuloma formation, collagen deposition, glandular atrophy, irregular cell proliferation; after paraffin embedding, serial sections were Masson (Masson) staining,analysis of image using Image-Pro Plus6.0 software, using Invert contrast inverseapplication photos, use the Photoshop into black and white gray, correction ofabsorbance using calibratio, intensity calibration, STD optical density, by analyzing the sedimentary area, the accumulation of collagen fiber optical density and average optical density, differential expression were compared between groups of collagen fibers. Analysis using SPSS17.0 software, the data measured by the mean standard deviation (x±s) said, two independent sample rate compared with x 2 test, compared among groups using single factor analysis of variance (one-way, ANOVA) were compared between the two two groups, the homogeneity of variance by using the method of LSD-t multiple comparison, if the homogeneity of variance, using Dunnett’sT3 multiple comparison. The small sample using Fisher exact test, measurement data using t test. a=0.05 is a statistical test standard.Results:On four groups of patients, Gender, age and other general information were analyzed. The results suggest differences gender differences in the groups was not statistically significant in terms (P>0.05). Age difference was statistically significant. The adenocarcinoma group has the highest average age (65.55±8.36) the normal group was the lowest (36.17±10.91) (P<0.01). The infection rate of Hp compared among the four groups was not statistically significant (P>0.05) normal control group which negative 3 cases, positive 3 cases, gastric ulcer group negative 9 cases, positive 11 cases, precancerous lesion group negative 6 cases, positive 7 cases, adenocarcinoma group negative 4 cases, positive 7 cases. The expression levels and deposition area of collagen fibers in adenocarcinoma are the highest of four groups (deposition area=108535.82±61071.51) (expression levels =29993.2041±16828.8527).the between each group of collagen fiber integrated optical density comparison (normal control-ulcer-dysplasia-cancer), were higher than that of normal control group and with the disease severity increased gradually but the difference was not statistically significant(P>0.05 sedimentary area (P value=0.106); the accumulation of P optical density>0.05 (P=0.135)). Adenocarcinoma has the highest intensity expressed of density of average potical (density of average potical=436.26±127.60) when the group of ulcer followed (nsity of average potical=374.28±124.77). the average optical density was not balanced and the difference was statistically significant (P<0.01 P=0.003).Conclusion:the expression of collagen fibers is difference in different gastric diseases. The expression of collagen fibers produce different changes will increase as a result of various factors and interventions in the antral mucosa. By observed the expression of collagen fibers which by Masson staining in different pathological conditions of antral mucosa. Collagen fibers increased meandering upward trend with the severity of the disease. The expression of collagen fibers is correlated with disease outcome amount.The second part:expression and relativity of pathology in gastric antrum mucosa of Ⅰ, Ⅲ,Ⅳ collagen and tgf-β1 and Hp infectionObjective:Explore the phase and morphology characteristics of development and progression of gastric cancer in different pathological tissues from the proportions of each of collagen by analysis the expression of collagen type Ⅰ, Ⅲ, Ⅲ antral mucosa histopathological, To investigate the correlation with Tgf-β1 expression.Materials and methods:select Zhujiang Hospital of Southern Medical University in 2013 November to 2014 March the pathology department of outpatient and inpatient antrum pathological specimens from patients and healthy subjects under gastroscope gastric antral mucosa tissue samples from 50 cases of normal,6 cases in the control group,20 cases of gastric ulcer,13 cases of precancerous lesion,11 cases of adenocarcinoma. Type Ⅳ collagen in the precancerous lesions in the group was the highest (sum=212256.92±57764.42), the lowest expression in gastric ulcer group (sum= 143975.67±38955.55), but no significant difference (P> 0.05 P 值=0.228). The highest expression of TGF-β1 in the normal control group (sum=153894.16±51428.16), the second is the precancerous lesion group (sum=144626.53±100117.19), expression level of gastric ulcer group and adenocarcinoma in the group is relatively low (sum gastric ulcer =107271.98±30403.97、sum adenocarcinoma=120607.17±32781.73), but no significant difference (P> 0.05 P 值=0.216). Compared the protein expression in each group within or without Hp infection, the expression intensity of type I collagen prompted that the uninfected oneexpression higher than who has been infected (Hp(-)315.40±293.27> Hp(+)310.36±272.71). Statistically significant (P<0.05, P=0.037) But with no significant in other compared.Conclusion:Ⅰ, Ⅲ, Ⅳ type collagen expression varies in different pathological states in the antral gastric mucosa, with the severity of the disease showed different regularity, closely related to the secretion of state may in various cells and TGF-β1, excluding the ulcer group. Hp infection has no statistically significant for the expression of proteins in each group, only showing a positive correlation in the trend of expression intensity with type Ⅰ collagen.
