| Objective: 1.Understand occurrence and influence factor of type 2 diabetes patients with osteoporosis.2.Discuss the clinical application value of calcaneal quantitat ive ultrasound on the diagnosis of osteoporosis. Methods: Collect the general information, biochemical indicators and occurrence of complications of endocrinology department in-hospital type 2 diabetes patients from December 2013 to May 2014, Dual-energy X-ray bone density instrument detection result was taken as the golden standard for analysis, the patients of 2-diabetes mellitus were divided into the osteoporosis group, non-osteoporosis group, and chi-square examination was applied for single-variable analysis of dangerous factors, and the logistic regression of binary variable was applied for multi-variable analysis, the occurring of osteoporosis of 2-diabetes mellitus patients was observed, and the possible influential factors were analyzed, and quantitative ultrasound bone density detection instrument was sued to detect the strength of the patients’ calcaneus, the characteristic curve of subjects was drawn, and the clinical application value of calcaneus quantitative ultrasound method in the diagnosis of osteoporosis was discussed. Results: Univariate correlation analysis shows that duration(OR=2.684), peripheral neuropathy( OR= 4.440), hypertension( OR= 2.281), coronary heart disease(OR=2.622) are in positive relation to type 2 diabetic osteoporosis(P<0.05), while BMI, high density lipoprotein are in negative relation to type 2 diabetic osteoporosis. Multivariate correlation analysis shows BMI(OR=0.311) is in negative relation to type 2 diabetic osteoporosis(P<0.05), peripheral neuropathy(OR=3.795) is in positive relation to type 2 diabetic osteoporosis(P<0.05). By calcaneal quantitative ultrasound detection method and dual-energy X-ray detection method, sensitivity are 44.44% and 34.99% for male, and female T2 DM patients with osteoporosis, while specificity are 97.22% and 99.17% respectively. In quantitative ultrasound detection, when T=-1.950, subject work feature curve lower area(0.807) is at its maximum, sensitivity is 57.7% while specificity is 89.3%. Conclusion: 1.2 type diabetic osteoporosis disease with a variety of factors. The course of disease, peripheral neuropathy, hypertension, coronary heart disease is the risk factors, body mass index(BMI), high density lipoprotein is the protective factors. 2.Compared with dual-energy X-ray, quantitative ultrasound has a certain diagnostic value in clinical. |
