Objectives: To assess the anxiety, depression and quality of live of patients with benign paroxysmal positional vertigo before or after their treatment. To observe the influence of Cognitive Behavioral Therapy on anxiety of benign paroxysmal positional vertigo.Methods: After Continuously including 59 patients of primary benign paroxysmal positional vertigo whose Hamilton Anxiety Scale(HAMA) score were greater than or equal 7, we completed the collection of history, acquisition of general information and diagnosis of BPPV of the patients. Cognitive Behavioral Therapy combined with otolith repositioning therapy were used in the study group, and the control group was treated with otolith repositioning therapy. The Hamilton Anxiety Scale( HAMA), Hamilton Depression Rating Scale(HAMD), and Dizziness Handicap Scale(DHI) were used to assess the patients in both groups at the their first visit and in the first, second, forth and eighth week after therapy.Results:(1) General information in both groups has no significant difference(P> 0.05).(2) The total short-term( one week) effective rate of posterior semicircular canal BPPV and horizontal semicircular canal BPPV are 93.33% and 93.10% respectively.(3)The total score, somatic anxiety factor score of HAMA of the study group are significantly lower than the control group in the first, second, forth,eighth week(P <0.01). The mental anxiety factor score of the study group are significantly lower than the control group in the first week(P <0.01).(4) The mean minus points of total score of HAMA in the study group are significantly higher than the control group in the first, second, forth, eighth week(P<0.01), and so as to the mean minus points of somatic anxiety factor(P <0.05). The mean minus points of mental anxiety factor are significantly higher than the control group in the first, forth, eighth week(P<0.05).(5) The total score, symptom factor score and psychological factor score of DHI in the study group are significantly lower than the control group in the firs week(P <0.05). The psychological factor score of DHI in the study group is significantly lower than the control group in the forth week(P <0.05).The total score and psychological factor score of DHI in the study group are significantly lower than the control group in the eighth week(P <0.05).(6) The mean minus points of total score of DHI in the study group are significantly higher than the control group in the first week(P <0.05), and so as to the mean minus points of total score of symptom factor(P <0.05), psychological factor(P <0.01) and social functional factor(P <0.05).Conclusions:(1) The Cognitive Behavioral Therapy combined with otolith repositioning therapy has better effect than otolith repositioning therapy on improving anxiety, especially the physical anxiety of BPPV.(2) The duration of dizziness disabilities of patients with acute BPPV shortens and the quality of life of them can be significantly improved by Cognitive Behavioral Therapy combined with otolith repositioning therapy. |
