Objective: Aim to compare the location of EH in unilateral Meniere’s Disease between c VEMP、o VEMP and Gadolinium-enhanced Inner-ear MRI.And to observe wether c VEMP and o VEMP match with MRI in the location of EH.Method: 3D-FLAIR MRI was performed at 24 h after an intratympanic injection of gadolinium diethylenetriamine penta-acetic acid in 69 unlateral MD patients recruited in the First affiliated hospital of fujian medical university vertigo clinic. Then the examination results were summarized, analyzed and compared with those of c VEMP and o VEMP.Result: 2 cases were ruled out, because they didn’t have successful Gadolinium-enhanced Inner-ear MRI performance.In 67 unlateral MD patients 95.5%(64/67)demonstrated the signs of endolymphatic hydrops in vestibular area.In 63 cases had completed c VEMP, c VEMP could be elicited in 44 ears and The percentages of patients with abnormal c VEMP is 52.3%。The sensitivity and specificity of c VEMP is 53.3% and 66.7% individually.o VEMP could be elicited in 55 ears, and the abnormalty rate is 32.3%. The sensitivity and specificity of o VEMP is 31.5% and 60% individually.Statistically significant differences existed between the results of 3D-FLAIR MRI and those of c VEMP and o VEMP(P<0.05).There are 3 cases only show saccule EH in MRI,one has abnormal c VEMP and normal o VEMP;one has normal c VEMP but abnormal o VEMP,another has normal c VEMP and o VEMP.There are 2 cases whoses MRI is normal show abnormal c VEMP in one and normal c VEMP in the other.Conclusion: The abnormalty rate of c VEMP and o VEMP are 52.3%and 32.3% individually.Both have their clinical value in evaluate saccule and utricle function, their result could be affected by a series of factors,so we think it’s not suitable to detect the location of EH by c VEMP and o VEMP only.It might be related with time intervals between VEMP and MRI. Compared with c VEMP and o VEMP,3D-FLAIR MRI after an intratympanic injection of gadoliniu has a higher value in detecting EH in saccule or(and) utricle in MD patients。...
