Cerebral Infarction Patients Hospitalized Costs Influence Factor Analysis And Prepaid Expenses Model Building

ObjectiveThrough investigate patients with cerebral infarction case information of jinzhong some hospital in 2014 years, research the main influencing factors of this disease hospitalization expenses, so as to put forward feasible suggestions.At the same time,use the stability influence factors to build a prepaid expenses of single diseases model, to provide the reference for the relevant administrative department of public health to formulate control cost and predict cerebral infarction disease health level of single macro-reforms lines.MethodsUsing retrospective survey research methods to study inpatient hospitalization expense of a three-level medical institution in jinzhong city, Shanxi Province, then collected patients’ basic information and medical records about the hospitalization expense.Using Epidata3.1 software package for data entry, application SPSS13.0 statistical software for data processing and analysis, taking frequency, mean, median and interquartile range of statistical description; using non-parametric test Kruskal-Wallis H Test and Mann-Whitney U Test for univariate analysis of hospitalization expense, multiple linear regression multivariate analysis was carried out on the hospitalization expenses. Logistic regression cost prediction model is established.ResultCerebral infarction patients hospitalization expense is 7931.8 Yuan per capita, the average length of stay was 13.8 days, medical payment is the main components of hospitalization expenses, accounting for 51.22%;Possible factors affecting hospitalization expenses for single factor analysis found that: age, medical payment, the situation on admission, hospitalization days, nursing way, NIHSS score level and the number of patients have influence on patients in hospital cost;Possible factors affectinghospitalization expenses for multiple linear regression analysis: age,the situation on admission, hospitalization days, nursing way, NIHSS score level and the number of accompanying disease are the main factors influencing of hospitalization expenses.Using Logistic regression Logit probability prediction model is established.ConclusionsAccording to the result of regression analysis to establish the optimal equation of cerebral infarction patients’ hospitalization expenses,age,the situation on admission,hospitalization days, nursing way, NIHSS score and the number of accompanying disease six factors associated with disease as the main factors influencing the patients’ hospitalization expenses; Using Logistic regression to establish cerebral infarction patients in hospital cost prediction model and the whole fitting is good.RecommendationThe government should increase financial input to the public hospital, improve patient health care reimbursement ratio, easing the burden on patients, focus from reduced patients’ hospitalization days, strengthen the rational use of drugs, reduce unnecessary tests,thus effectively control the hospitalization cost growth, realize the public welfare nature of public hospitals. The health administrative department shall be according to the illness needs to set up the reasonable level of single macro-reforms, solve the problem of single disease payment.
