Objcctive:To investigate the effects of etomidate on rat renal cell apoptosis and Bcl-2/Bax protein expression underwent hepatic ischemia reperfusion injury. Methods:Fifty-four male Wistar rats were randomly divided into 3 groups:sham operation group(sham group, n=18), ischemia-reperfusion group(I/R group, n=18), etomidate group(Eto group, n=18). Each group was divided into 3 time phases at 2h, 4h, 6h after reperfusion. To establish a rat model of hepatic ischemia-reperfusion. Except the sham group, other groups experienced liver ischemia of 60 minutes and then recovered reperfusion. Sham group and I/R group was pumped 0.9% saline(0.1 ml·kg-1·min-1)10minutes after liver ischemia of 50 minutes. Eto group was pumped etomidate(0.1ml·kg-1·min-1) 10 minutes. The left kidney were sampled at the end of reperfusion and observed by morphology and hematoxylin and eosin(HE) staining. The histological change of kidney were observed using light microscopy. Cys C was measured by biochemical analysis.Bcl-2 and Bax protein expression in kidney tissue was assessed by immuno-histochemical method. The apoptosis index(AI) of kidney cell were measuered by TUNEL mothod. Results:(1)Tunel staining showed: compared with Sham group, the AI increased both in group I/R and Eto(P<0.05), with statistical significance. Compared with I/R group, the AI decreased in group Eto(P<0.05),with statistical significance. At the same ischemia time point, the AI increased successively in group Sham, Eto, I/R and there exists maximum value in group I/R 6h.(2)Compared with Sham group, the expression of Bcl-2 and Bax protein increased both in group I/R and Eto(P<0.05), with statistical significance. Compared with I/R group, the expression of Bax protein decreased and Bcl-2 protein increased in group Eto(P<0.05), with statistical significance. At the same ischemia time point, the expression of Bcl-2 increased successively in group Sham, I/R, Eto and there exists maximum value in group Eto 6h; The expression of Bax increased successively in group Sham, Eto, I/R and there exists maximum value in group I/R 6h.(3)Compared with Sham group, the content of Cys C increased both in group I/R and Eto(P<0.05),with statistical significance. Compared with I/R group, the content of Cys C decreased in group Eto(P<0.05), with statistical significance. At the same ischemia time point,the content of Cys C increased successively in group Sham, Eto, I/R and there exists maximum value in group I/R 6h.(4)The pathologucal lesions were lighter in group Eto than that in group I/R under light microscopy. Conclusion:Etomidate has a protective effect on renal cell apoptosis by hepatic ischemia-reperfusion, its mechanism may be increasing Bcl-2 level and reducing Bax level.
