| Objective:Use two-dimensional contrast-enhanced ultrasound and three-dimensional contrast-enhanced ultrasound technology to observe and analyse the gallbladder lesions’ perfusion characteristics,enhancement pattern and the overall shape. To evaluate the diagnostic value of 2D-CEUS and 3D-CEUS in benign and malignant lesions of the gallbladder.Methods:Fifty-two patients with suspected gallbladder lesions were examined with contrast-enhanced two- and three-dimensional ultrasonography. The imaging characteristics of benign and malignant gallbladder lesions were compared and analysed. Sonographic imaging findings were compared with the pathological results.Resluts:(1) In the 2D-CEUS, 9 cases gallbladder lesions which conventional ultrasound can not distinguish showed no enhancement(100%). The washout time showed significant different between benign and malignant gallbladder diseases(P<0.05).The time of arrival enhancement and the time to peak between benign and malignant group had no significant difference(P> 0.05).It was significant different between benign and malignant gallbladder diseases(P<0.05), including the lesion enhanced morphology, base width, the degree of enhancement in later arterial phase and the intactness of gallbladder wall. The enhancement extent in early arterial phase had no statistical differences(P> 0.05).The sensitivity, specificity and accuracy of 2D-CEUS to dignose benign and malignant gallbladder diseases are 92.31%ã€97.44% and 96.15%.(2)In the 3D-CEUS, 9 cases gallbladder lesions which conventional ultrasound can not distinguish showed no enhancement(100%). It had statistical differences between benign and malignant gallbladder diseases(P<0.05), including the size of the lesion, internal enhancement, enhanced morphology, base width and cystic wall boundary. The sensitivity, specificity and accuracy of 2D-CEUS to dignose benign and malignant gallbladder diseases are 92.31%ã€94.87% and 94.23%.(3)Comparing, the diagnostic value of 2D-CEUS and 3D-CEUS both are high.The diagnostic value had no statistical differences(P>0.05).Conclusions:Contrast-enhanced two-dimensional sonography has an advantage in showing perfusion characteristics of blood flow of benign and malignant gallbladder lesions.Contrast-enhanced three-dimensional sonography could display the angiographic characteristics of the gallbladder diseases. Both contrast-enhanced two-and three-dimensional sonography have high diagnostic value in the diagnosis of benign and malignant gallbladder lesions. |
