Objective: This study examines the Th1/Th2 balance and EGF expression in rats with chronic atrophic gastritis(CAG) as experimental animal models. By using results of laboratory tests of the pathological histology and specificity, this study investigates the influence of Shengyang Yiwei Decoction on atrophic gastritis immunity response to Th1/Th2 balance and EGF expression and discusses the effects and mechanism of this treatment on CAG. The study also helps us get to know and control the disease from the view of the origin of the universe- Yin and Yang. In the aspect of treatment, the study creates a new research direction of the treatment of the disease with traditional Chinese medicine by comparing the pharmacodynamic effects of Chinese and western medicine on CAG in rats.Methods: Use 121 adult SPF healthy Wistar rats of both genders weighing between 180 and 220 grams each to model the CAG rats based on the synthetic method, feed them regularly for 3 days. Then select 51 rats randomly as treatment group,The remaining 70 for prevention group.From 70 prevention group rats,select 10 rats randomly as prevention normal group and feed them regularly. The remaining 60 rats were stimulated by multiple methods including immunostimulation, ammonium hydroxide, alcohol, salicylic acid, and deoxysodium cholate, together with irregular diet to model the CAG rats for 10 weeks. From the beginning of the 7th week, divided the rest of rats to 5 groups randomly numbered by their weights, namely, prevention model group(II), prevention control group(III), low-dose traditional Chinese medicine prevention group(VI), medium-dose traditional Chinese medicine prevention group(V), high-dose traditional Chinese medicine prevention group(IV). While modeling, intervening low-dose traditional Chinese medicine prevention group,medium-dose traditional Chinese medicine prevention group, high-dose traditional Chinese medicine prevention group with Shengyang Yiwei Decoction by filling the stomach; and intervening prevention control group with sucralfate by filling the stomach.In the 10 th week,stop making mould at the same time. After successfully modeled, kill rats.In the same molding method,51 treatment group rats were stimulated to 10 weeks. After modeling,divided the rest of rats to 4 groups randomly numbered by their weights,namely,low-dose traditional Chinese medicine treatment group(IX), medium-dose traditional Chinese medicine treatment group(VIII), treatment model group(X),high-dose traditional Chinese medicine treatment group(VII). Treating low-dose traditional Chinese medicine prevention group, medium-dose traditional Chinese medicine prevention group, high-dose traditional Chinese medicine prevention group with Shengyang Yiwei Decoction by filling the stomach.After 4 weeks of treatment,kill rats.In the 10 th week, select 5 rats randomly, respectively from prevention model group and treatment model group.Then kill and collect samples of their gastric tissues to confirm the model. After successfully modeled, kill all the prevention group rats,sample their femoral artery blood, cut and weigh spleen and thymus, and collect samples of their gastric tissues.At the end of the 14 th week, kill all the treatment group rats, sample their femoral artery blood, cut and weigh spleen and thymus, collect samples of their gastric tissues.Calculate spleen index and thymus index; radioimmunoassay is used to examine the volume of IL-2,IFN-γ, IL-4 and IL-6 in gastric tissues of the rats in all groups, then calculate IFN-γ/IL-4 to observe their immunity response to Th1/Th2 balance. Immunohistochemistry is adopted to examine the epidermal growth factor(EGF) expression of the gastric tissues.Results:1.Spleen index and thymus indexCompared to rats in the prevention normal group, spleen index and thymus index of rats in prevention model group spleen index and thymus index decreases considerably(p<0.05),indicating the success of modeling; compared to rats in the prevention model group, the spleen index and thymus index of prevention control group and treatment groups with different doses of traditional Chinese medicine are different(p<0.05); compared to rats in prevention control group, spleen index and thymus index of rats in all prevention groups are different(p<0.05), among which the medium-dose group produced the most significant rise.Compared to.rats in the treatment model group, rats in treatment groups with different doses of traditional Chinese medicine are are different in their rise(p<0.05), among which the high-dose group produced the most significant rise.2.Effects of Shengyang Yiwei Decoction on IL-2, IFN-γ, IL-4,IL-6 and IFN-γ/IL-4 in gastric tissue serum of the rats by using radioimmunoassayCompared to rats in the prevention normal group, secretion of IL-2,IFN-γ and IFN-γ/IL-4of rats in the prevention model group decreases considerably and the secretion of IL-4 and IL-6 increases significantly, both of which are of statistical significance(p<0.05). Compared to rats in the prevention model group, the rats in the prevention control group and prevention groups with different doses of traditional Chinese medicine secreted considerably more IL-2,IFN-γ and IFN-γ/IL-4 and less IL-4 and IL-6 on statistical levels(p<0.05), among which the medium-dose group produced the most significant changes.Compared to the rats in the prevention control group, the rats in the medium-dose prevention group produced more IL-2,IFN-γ and IFN-γ/IL-4 and less IL-4 and IL-6, which is statistically significant(p<0.05). Compared to rats in the treatment model group, the rats in all treatment groups with different doses of traditional Chinese medicine produced more IL-2,IFN-γ and IFN-γ/IL-4 and less IL-4 and IL-6, which is statistically significant(p<0.05).Among all traditional Chinese medicine treatment groups, the rats in the high-dose treatment group produced more IL-2,IFN-γ and IFN-γ/IL-4 and less IL-4 and IL-6, which is statistically significant(p<0.05).3. Results of the volume of EGF in gastric tissues based on immunohistochemistryCompared to rats in the prevention normal group, the increase of EGF expression in rats in the prevention model group is statistically significant(p<0.05), indicating the success of modeling; compared to rats in prevention model group, the EGF expression in rats in the prevention control group and all prevention groups decrease considerably, which is statistically significant(p<0.05). Compared to rats in prevention control group, the EGF expression in rats in the medium-dose and high-dose traditional Chinese medicine prevention group decrease considerably, which is statistically significant(p<0.05), among which the medium-dose prevention group produced the most significant decreases;compared to rats in the treatment model group, the EGF expression in rats in all treatment groups decrease considerably, which is statistically significant(p<0.05); compared to rats in the low-dose traditional Chinese medicine treatment group, the EGF expression in rats in high-dose and medium-dose treatment groups decrease considerably, which is statisticallysignificant(p<0.05), among which the high-dose treatment group produced the most significant decreases.Conclusion:1. The experiment produces successful results by using this comprehensive modeling method to model CAG on rats.2. The prevention and treatment of rats with Shengyang Yiwei Decoction can increase spleen index and thymus index considerably, and increase immune function significantly.Using Shengyang Yiwei Decoction as an prevention medicine and treatment can increase the secretion of IL-2,IFN-γ and IFN-γ/IL-4 and decrease the secretion of IL-4 and IL-6 in the gastric tissue and serum of rats with CAG. It can also reduce EGF expression in gastric mucosa of these rats. Among the results, the benign data from the medium-dose prevention group and high-dose treatment group are the most obvious, both of which are statistically better than the data from the sucralfate control group.3. Shengyang Yiwei Decoction producing obvious results in medium-dose prevention and high-dose treatment fully reflects the thought of traditional Chinese medicine, namely,“giving priority to prevention and preventing diseases from exacerbating”, and proves the dose-effect relationships among all groups.4. The research also shows that Shengyang Yiwei Decoction produces better treatment on model rats with CAG and increases the immune mechanism by raising the spleen index and thymus index. This mechanism increases the secretion of IL-2,IFN-γ and IFN-γ/IL-4,decreases the secretion of IL-4 and IL-6 and reduces the EGF expression in gastric mucosa,so it could achieve the purpose of curing CAG by alleviating gastric mucosa damages in CAG rats and improving the recovery of gastric mucosal lesion.
