Objective:With the continuous development of clinical medicine, artificial total hip arthroplasty total hip arthroplasty(THA) applications more and more widely. Many youth were more likely to have biological handle artificial total hip arthroplasty, relatively speaking, bone cement handle more application in elderly patient populations, however, the efficacy of two different materials prosthesis scholars are different views, there are many disputes. Is the object of study in our hospital from January 2012 to January 2013 with the artificial hip joint replacement patients 1 82 patients by retrospective analysis of the clinical data, in order to efficacy of bone cement handle and biological handle in elderly patients compared evaluation of the artificial hip joint replacement in two handle the clinical curative effect, and then combined with the research results, clearly the usage of the two handle, and comparing the domestic and foreign research and the shortcomings of the existing in our country put forward effective suggestions for improvement.Methods:A retrospective analysis of the First Affiliated Hospital of Nanchang University from January 2011 to January 2013 using artificial total hip arthroplasty(THA) of 182 cases of elderly patients(70 years), including 89 cases, 32 cases of aseptic necrosis of the femoral head, degenerative arthritis of the hip in 61 cases of femoral neck fracture. According to the different types of prosthesis, the patients were divided into bone cement handle group 45 cases: 25 cases were male, 20 cases were female, age 71 years to 95 years old,biological handle group 137 cases: 79 cases were male, 58 cases were female, age 71 to 98 years old. During the peri operation period and evaluation of two groups of patients with average operation time, average bleeding volume, average rate, mean postoperative start loading time, the average length of time and operative and postoperative complications; during the follow-up period and evaluate the loosening of the two groups of patients with Harris score, prosthesis loosening.Results:1.Perioperative period, two groups of average operative time, intraoperative complications occur cardiovascular events in bone cement reaction by comparison, the difference has statistical significance(P < 0.05), while the two groups the average intraoperative bleeding volume, average rate, mean postoperative start loading time, average length of stay in hospital time, postoperative complications by comparison. There were no significantly differences were statistically significant(P > 0.05). 2.follow-up period, two groups of patients with postoperative 1, 6, 12, 24 months Harris hip score differences do not have statistical significance(P > 0.05). In the bone cement handle group, after 6 months of Harris hip score is better than that of postoperative 1 month(P < 0.05); 1 year postoperative Harris hip score is better than that of postoperative 1 month(P < 0.05); compared to the postoperative 6 months Harris hip scores and patients after 12 months, the difference is not statistically significant(P > 0.05). In biological handle group, after 6 months of Harris hip score is better than that of postoperative 1 month(P < 0.05); after 12 months of Harris hip score is better than that of postoperative 1 month(P < 0.05); compared to postoperative 6 months hip scores and patients after 12 months, the difference is not statistical significance(P > 0.05). There was no postoperative loosening of the prosthesis in the two groups, the difference was not statistically significant(P>0.05)Conclusion:In elderly patients, the biological group of patients operating handle shorter operative time, complications occurred without bone cement, Comparing cemented stem group has certain advantages,There was no evident difference compared of the two groups of patients with average average bleeding volume, average rate, mean postoperative start loading time, the average length of time,postoperative complications and prosthesis loosening. In elderly patients with THA, bone cement handle and biological handle can obtain good clinical curative effect,Need to conduct a comprehensive assessment surgery for patients in general,and combined with the operation of surgical skills and proficiency to choose suitable for elderly patients with artificial joint prosthesis. |
