Objective: Retrospectively investigate clinical data of catheter related bloodstream infection for tunneled cuffed catheter in maintenance hemodialysis patients of nephrology ward in the Affiliated Zhongshan Hospital of Dalian University to prevent the occurrence of catheter related bloodstream infection and reduce the mortality rate Methods: 130 hemodialysis patients with tunneled cuffed catheters and treated in nephrology ward in the Affiliated Zhongshan Hospital of Dalian University during February 2013 to February 2015 were enrolled in this study. Their clinical data and the prevalence, distribution of bacteria, treatments and clinical outcome were analyzed. Results: In the hemodialysis patients,the total cumulative duration of indwelling catheterization was 11,076 days,the median duration of indwelling catheterization was 320 days( 35-1965days). CRBSI was found in 33 cases with the prevalence of 25.3%.The prevalence of CRBSI was significantly associated with duration of catheterization. Gram-positive bacteria were detected in 22 cases, and Gram-negative bacteria in 10 cases(66.7%å’Œ 30.3%, respectively; P>0.05).The main bacterias were Staphylococcus aureus(30.3%), Pseudomonas aeruginosa(12.1%),Staphylococcus epidermidis(15.2%)ã€methicillin-resistant Staphylococcusaureus(12.1%),Escherichia coli(6.1%) and Enterococcus faecalis(9.1%).The prevalence of CRBSI in patients with subclavain vein catheterization was the lowest,the follow was in those with jugular vein catheterization,the highest was in those with femoral vein catheterization. Conclusion: In our blood purifying center, the prevalence of CRBSI hemodialysis patients in those with TCC was significantly associated with duration of catheterization Gram-positive bacteria and Gram-negative bacteria was not significantly variable( P>0.05).The prevalence of CRBSI hemodialysis patients in those with TCC was significantly variable in different catheter sites. All CRBSI patients was given systematic antibiotics and use of antibiotic lock solutions with UK.The total cure rate was 99.2%。... |
