| Objective: The study aimed to investigate the effect of Biofeedback electrical stimulation therapy improving Pelvic Floor Dysfunction of Total Hysterectomy.Methods : In May 2013-May 2014 at the First Affiliated Hospital of Medical College,Shihezi University,Obstetrics and Gynecology, A randomized collected due to Benign uterine disease patients Requiring hospitalization with total hysterectomy. Collectiing general information, Filingl in PFIQ-7. PISQ-31 and ICI-Q-F,Learning pelvic floor muscle by gynecological examination,Confirming no merger SUI before Hysterectomy by Ultrasound of three-dimensional pelvic floor and Urodynamics.Begining to follow up Patients after one months of operation,Survey the number of patients with occurred urinary incontinence and Degree of urinary incontinence Postoperative,collect quantitative indicators valueof pelvic floor muscle strength.PFIQ-7.PISQ-31 in all patients with incontinence,By Voluntary principles Individualized Biofeedback therapy and kegel training gived. the control group is Kegel exercise alone.Pelvic quantitative indicators value of other healthy control group compares with incontinence pelvic floor group;Postoperative patients is followed up for 6 months again, collect and compare the degree of both groups’ s improvement in Pelvic floor function and Various quantitative indicators value after treatment.Result:Randomly collecting 80 cases of Healthy patients who are Comparable in terms of Age, body mass index,initial fetal birth weight, mode of delivery and quantity,menopause situation,is normal control. According to the principle of voluntary Urinary incontinence of patiens is divide into Treatment group(Biofeedback and kegel training group) and the control group(Kegel exercise alone group). By comparing the pelvic floor related parameters of three groups one month after operation:Three groups were differences between the pelvic floor muscles, urinary incontinence group was significantly weaker than the normal control group, and the difference was statistically significant;according to Urodynamic parameters,Urinary incontinence group and normal control group have differences in Qmax, urination peak time(TQmax), urinary flow time(VT), maximum urethral pressure(MUP), maximum urethral closure pressure(MUCP),the difference was statistically significant;Comparison of three-dimensional pelvic ultrasound parameters derived that Bladder neck mobility and Urinary bladder posterior horn are different, and the difference was statistically significant.six month after operation:Pelvic floor muscle was significantly increased both therapy group and the control group;Comparison of urodynamic results that treatment group in the maximum urethral pressure(MUP), maximum urethral closure pressure(MUCP), abdominal leak point pressure(ALPP) is large than the control group, and the difference was statistically significant;In bladder neck mobility,the distance of Bladder neck down to under pubic symphysis and posterior vesicourethral angle therapy group is less than the control group,the difference was statistically significant.Conclusion: Biofeedback therapy can be more effective in improving the symptoms of female pelvic floor dysfunction after surgery,the feature has a simple, easy to use, no side effects,biofeedback therapy is worth widely used. the principle of individual treatment should be emphasized in practical applications, and it mobilize the patient to actively participate with the treatment. |
