Research Object1.To study the diagnostic value of conventional ultrasound in benign and malignant single solidthyroid nodules identification.2.To study the diagnostic value of real-time tissue elastograph(RTE)in benign and malignant singlesolid thyroid nodules identification, which mainly refers to the five scoring method of real-time tissueelastograph.3.To study the diagnostic value of acoustic radiation force impulse(ARFI) in benign and malignantsingle solid thyroid nodules identification.4.To compare the diagnostic value of above three methods of identification of benign and malignantsingle solid thyroid nodules.Materials and Method100 patients with 100 solitary thyroid nodules were examined by conventional ultrasound andultrasonic elastography imaging from December 2013 to July 2014 in Directly Affiliated Hospital ofShihezi University School of Medicine. The typical cystic masses and the cystic—based mixed tumorswere not included. All solid nodules were removed by surgeons and were done histopathologicalexamination. Male 34,female 66,the age range is from 21 years old to 72 years old. The average age is(47.6±10.1)years.Conventional ultrasound to observe preliminary information,including itsshape,edge,size,capsule,aspect ratio, echo intensity,calcification and lateral shadow;RTE adopt a flexibleclassification method,And ARFI detecte by tissue lesions(SWV), pathological diagnosis as the goldstandard for evaluation of the diagnostic value of three methods.ResultsThe ROC curve was drawn according to the SWV.The area under the curve was 0.960,theYoudenindex of SWV was 2.82.The sensitivity,specificity,PPV and NPV of US,UE and ARFI respectivewas 82.61%( 19/23),87.01%( 67/77),65.52%(19/29),94.37%(67/71) and 82.61%( 19/23),88.31%( 68/77),67.86%(19/28),94.44%(68/72) and 91.30%( 21/23),93.51%( 72/77),80.77%(21/27),97.29%(71/73).Conclusions1.Using conventional ultrasound, ultrasound elastography score, acoustic radiation forceimpulse(ARFI) technology to identify benign and malignant single solid nodules of the thyroid all have acertain diagnostic value.Conventional ultrasonic technology is a foundation, ultrasound elastography is auseful complement to conventional ultrasound diagnostic.2.The ability of identifying thyroid malignant single solid nodules by ARFI technology is better thanthat by conventional ultrasound and ultrasound elastography score.The ARFI technology can improve thediagnostic sensitivity of malignant nodules and early thyroid cancer detection rates and can possibly realizethe early detection, early diagnosis, early treatment. The ARFI technology worth clinical promotion.3.The determination of SWV value can quantitative reflect the hardness of the lesions. It is moreobjective than ultrasound elastography classification in judging the benign and malignant thyroid singlesolid nodules.But they are affected by many factors, so comprehensive judgment is required in the clinicalapplication of work.
