| Objective To investigate the clinical value of the Cai tube used in laparoscopic-assisted natural orifice specimen extraction(LA-NOSE) of colorectal resection.Methods From January 2014 to February 2015, 23 cases of colorectal diseases under total laparoscopic colorectal resection surgery with the LA-NOSE according to cancer treating principle. Observing the time of operation, blood loss, postoperative recovery and following up to assess the short-term effect.Results The 23 cases were successfully completed in total laparoscopic dissection and anastomosis with no deaths, no convertion to laparotomy, no anastomotic bleeding, no fistula, no stenosis or any other complications. The Postoperative pathology of 18 cases were adenocarcinoma,1 case was rectal carcinoid and 1 case was mucinous adenocarcinoma,the cutting margin of the 20 cases were both cancer-free. 3 cases of patients were benign lesion.The average time of operation was(247±69) min. The average amount of bleeding was(81.7±62.9)ml. The average of lymphadenectomy cleared was(11.4±4.9) pieces. Recovery of gastrointestinal function range from 1~4d after operation, length of stay average of(13.1±3.1) d. 23 cases were followed up for the period of 2 ~ 15 months, no local recurrence, metastasis, wound planting or other complications.Conclusion The Cai tube used in auxiliary incision free of total laparoscopic colorectal resection with the LA-NOSE is safe and feasible in the clinic, with good cosmetic results, less trauma, less bleeding, lighter postoperative pain, quicker recovery. Its recent results were satisfactory, however, long-term complications andefficacy of oncology require an in-depth further study. |
