Objective:1. To screen the indicators which could be reflected the condition of complex acid-base disturbance in the patients with acute respiratory failure(ARF) and served as prognostic marker;2. To evaluate the probability that strong ion gap was selected as predictive factor of mortality in patients with acute respiratory failure.Methods:1. 106 patients with acute respiratory failure were collected which conform to the diagnostic criteria of acute respiratory failure, in which 60 cases were male and 46 cases were female with age from 13 to 88 years old(54y±17). Meanwhile, 106 cases were selected as matched control during the same period and gender, the similar ward and age(aged range from 16 to 92 years old, 55y±18).2. Acid-base disturbance indexs of K+、CL-、HCO3-、AG、p H、ALB 、SIG、Cr、lactic acid、Pa CO2、Pa O2、Na+、PO4- was detected.3. All the data were statistically analyzed by t-test and ROC curve was used to evaluate the disease development.Results:1. The incidence of acute respiratory distress syndrome and source pulmonary edema was 26% respectively in patients with ARF. Once ARF occurred, its mortality could be reached to 50%.2. There was a significant difference in the values of K+、CL-、HCO3-、AG、p H、ALB 、SIG、Cr、lactic acid、Pa CO2 and Pa O2 between group T1 and control. However, there is no significant difference between recovery group T2 and control.3. There was a significant difference in the value of AG and SIG between the recovery group T1 and death group T1 from the patients with ARF.4. SIG was a better predictor than AG by the evaluation of ROC curve.Conclusions:AG and SIG can reflect the worse condition of acid-base disturbance in patients with ARF sensitively at the first beginning. Meanwhile, SIG may be served as a predictor for risk of mortality in patients with ARF better than AG.
