Objective:Comparison of two vaginal preparation on postoperative vaginal microflora. Assess total hysterectomy vaginal microflora recovery. Methods:Select patients need to line 40 of hysterectomy were randomly divided into metronidazole group(20 cases) and povidone-iodine group(20 cases). Metronidazole group:(metronidazole Grinding) 3 days before surgery metronidazole vaginal cotton scrub, then metronidazole powder 2g placed posterior fornix, 1 day, 3 days in a row. Povidone-iodine group: 3 days before surgery iodine cotton scrub vagina once a day for three days. Taken at different times to evaluate the patient’s vaginal secretions vaginal microflora environment; 1st: pre vaginal preparations; 2nd: 3rd vaginal preparations, stopping one day, the next day vaginal discharge; third: One week after hysterectomy; fourth: total hysterectomy two weeks; fifth: total hysterectomy three weeks; sixth: total hysterectomy four weeks; seventh: hysterectomy resection February. Vaginal p H value two groups were compared, vaginal cleanliness, the detection number of lactobacilli, bacteria density and diversity. Results:1. Comparison of two different drug: drug before the two bacteria density, diversity, cleanliness difference was not statistically significant(c2=0.000,p=1.000;c2=0.4762,p=0.4902;c2=0.4396,p=0.5073), the two groups after intensive medicine, diversity, cleanliness no difference(c2=0.2778,p=0.5982;c2=0.000,p=1.000;c2=0.000,p=1.000).2. Vaginal p H value comparison: vaginal p H <4.5 constituent ratio on the drug before, after one week, after two weeks, after three weeks, after 4 weeks and 2 months after each 82.5%, 10%, 12.5 %, 27.5%, 47.5%, 70%, before and after the p H value on the drug each time compare the differences were statistically significant. No significant difference between the two groups.3. 3. Comparison of the intensity of vaginal flora: on the drug front vaginal flora intensity of class II-III ratio was 85%, total hysterectomy one week, two weeks, three weeks, four weeks, the proportion was 10 February %, 17.5% 4 7.5%, 55%, 70%..4. Comparison of vaginal flora diversity: front vaginal flora diversity on the drug class II-III 72.5. After 1 week, 2 weeks several species did not reach above the level II, after three weeks, four weeks and after February proportions were 15%, 20%, 35%. Preoperative and postoperative differences were statistically significant in each period, no significant difference between groups. 5. Lactobacillus detection rate: hysterectomy before resection, one week, two weeks, three weeks, four weeks and 2 months after lactobacilli detection rate of 100%, 0,5%, 20%, 45%, 65%. Vaginal lactobacilli detection rate before and after each period of preoperative detection rate statistical difference; no significant difference between groups. Conclusions:1. Iodine and metronidazole vaginal preparation can improve vaginal cleanliness, no significant difference between the two. 2. Prepare iodine and metronidazole vaginal vaginal microflora after excision of the whole uterus was no significant difference. 3. The total hysterectomy vaginal microflora Declining: vaginal p H value increased, the detection rate decreased lactobacilli, bacteria and bacterial diversity intensity decreased; total hysterectomy vaginal microflora self-recovery tendency, after the first three weeks began to recover.
