The Clinical Characteristics And The Treatment In Perioperation Period Of The Senile Patients With Meningioma

Objective:Through the research of clinical features, treatment of peri operation and complications of the 67 cases of elderly patients with meningiomas admitted to our department from February 2010 to February 2015 in our department,so as to improve the operation effect of elderly patients with meningioma and reduce the postoperative complications.Methods:The clinical data of 67 senile patients with menigiomas,over the age of 60,who underwent microsurgery from February 2010 to February 2015 in our hospital,were retrospective analyzed. The perioperative gists and the measures of preventing and treating the postoperative complications were discussed.The clinical characteristics of senile patients with meningiomas, peri operation period of treatment and prevention of complications were analyzed.Result:1.According to Simpson grade:40 cases of grade I resection, 8 cases of grade II resection, 3 cases of grade III resection, 3 cases of grade IV resection. 2 dead cases after the surgery(1 case died of respiratory failure caused by infection,1 case died of intracranial hematoma).2.After the surgery:4 cases of hemiplegia,6 cases of intracranial hematoma,4 cases with cerebral infarction,2 cases of aphasia,3 cases of intracranil infection,4 cases of epilepsy,13 cases of pulmonary infection,incision infection in 4cases,5 cases with arrhythmia,1 case with renal failure,2 cases of deep vein thrombosis,2 cases of hemorrhage of upper digestive tract.3.Follow-up examination 3 months to 5years:1 case of mild hemiplegia,2 cases of aphasia,3 cases with visual impairment,2 cases of loss of sense of smell,4 cases with tumor recurrence or residual tumor growth:1 case with meningeoma of parasaggital sinus,1 case of sellal meningioma,l case with inside sphenoid wing meningeoma,2 cases of petroclival meningioma.4.According to the related factors of complication during perioperation perid of the elderly patients with meningioma to actively prevent and treatment in our department from the beginning of June 2012,the incidence and mortality of various complications were decreased significantly.Conclusion:1.The characteristics of the senile patients with meningioma is insidious onset,long course of disease,the clinical manifestation is not typical,easily misdiagn-osed.2.Operation is the preferred method of treatment of senile patients with meningiomas.3.The function of each organ system of the elderly have different range decline,and is often associated with underlying diseases,the positive and compreh-ensive treatment in perioperation period can effectively reduce the incidence of postoperative complications,improve the efficacy and quality of life in patients with operation.
