Curative Effect Of Treatment By Partial Nephrectomy And Radical Nephrectomy For Stage T1 Renal Cancer Were Retrospectively Analyzed

Objectives To compare the treatment effect of partial nephrectomy with that of radical nephrectomy for stage T1 of renal cell carcinoma, which would help to provide theoretical basis for the choice of its further treatment.Methods From January 2000 to June 2014, data of 180 patients with stage T1 renal cell carcinoma in our hospital and the affiliated hospital of Hebei medical university were screened, who had been respectively given partial nephrectomy and radical nephrectomy,including 68 cases of patients being given partial nephrectomy and 112 cases of patients being given radical nephrectomy. The relevant information included the gender, age,presence of preoperative clinical symptoms, operation time, intraoperative blood loss,postoperative complications. And according to the follow-up situation, it would be understood that their 5-year specific survival rate, 5-year without event survival rate and5-year survival rate of no transfer. The efficacy of partial nephrectomy and radical nephrectomy would be evaluated for T1 renal cell carcinoma by comparing these data being sorted out and analyzed. All of the data would be analyzed by SPSS 17.0 software,such as independent sample t-test for measurement data, χ2-test for count data, and logrank test for survival rate. The difference was statistically significant as P<0.05.Results The average age of the two groups of patients were partial nephrectomy(54.3± 10) and renal cell carcinoma radical resection(54.3±10) years of age, gender ratio of partial nephrectomy and radical nephrectomy in 55.9% the proportion of male male ratio of 53.6%, and no preoperative clinical symptoms were partial nephrectomy without clinical the symptoms was 64.7% and the radical nephrectomy without clinical symptoms accounted for 66.1%, no statistical significance(P > 0.05). The operation time of the two groups were partial nephrectomy(132.4±13.6) min lymphadectomy and renal cell carcinoma(109.1±14.7) min, the comparison between the two, there are obvious differences, with statistical significance(P<0.01). The amount of bleeding of the two groups were partial nephrectomy(142±109.6) ml lymphadectomy and renal cell carcinoma(125.2±96.2) ml, the comparison between the two, there are obvious differences, with statistical significance(P<0.05). Two groups of patients with preoperative creatinine partial nephrectomy(75.8±5.9) mol/L and radical resection ofrenal cell carcinoma(76±5.4) mol/L, no statistical significance, but after 1 months,1years, 2years, 3years, there are some obvious differences, with statistical significance(P<0.05). 5years specific survival rate of the two groups of patients were partial nephrectomy and radical nephrectomy in 95.5% and 97.1%,respectively,no significant difference,no statistical significance(P>0.05). Two groups of patients with 5 year event free survival rate was 91.2%and partial nephrectomy and radical nephrectomy in91.1%,in comparison, no significant difference, no statistical significance(P>0.05). Two groups of patients with 5 year metastasis free survival were 95.6% partial nephrectomy and radical nephrectomy in 99.1%,in comparison, no significant difference, no statistical significance(P>0.05).Conclusion 1.For T1 stage renal cell carcinoma patients, partial nephrectomy can more nephron and renal function, reduce the incidence of postoperative renal failure, help to improve the long-term survival of the patient.2. For stage T1 patients with renal cell carcinoma had no obvious effect, part of renal resection and radical nephrectomy 5 years survival rate and postoperative complications.3 Effect of amount of bleeding and operation time were mainly in the surgeon’s experience and tumor complexity, suggesting that partial nephrectomy than traditional radical nephrectomy were complex, high requirements for operation skills.
