Object:To analyze the postoperative cancer specific survival rate、recurrence rate、progression free rate and the quality-of-life of the Primary T2、T3 muscle invasive bladder cancer Patients who were treated of partial cystectomy combined with adjuvant chemotherapy of GC bladder preserving therapy and underwent radical cystectomy and find the factors influencing the effect of postoperative survival rate.Methods : Retrospective the bladder cancer patients who had surgical operation in the first affiliated hospital of Dalian Medical University from January2012 to January 2015.Among the patients,15 patients performed partial cystectomy combined with GC adjuvant chemotherapy therapy and 62 patients performed radical cystectomy.Adjuvant chemotherapy of GC bladder preserving therapy was performed after surgery in patients with the partial cystectomy. The patients who did not have pathological report and were metastatic are excluded.Among the followed-up patients,53 cases were primary tumor and 24 were recurrent tumor.77 patients were followed up,62 cases of patients performed radical cystectomy,and 15 patients performed partial cystectomy. All patients who performed partial cystectomy were confirmed by pathological negative margin,no incision metastasis.The postoperative pathological stage T2 patients were 55 cases,22 casesof T3 patients.Follow-up the postoperative survival rate, recurrence rate and the long-term quality of life of patients who performed partial cystectomy and radical cystectomy and analyze the follow-up date.Compare the difference in survival rate 、 recurrence rate and the postoperative quality of life influence between the muscle invasive bladder cancer patients who performed the two kinds of operation mode.Results:In 97 muscle invasive bladder cancer patients,77 cases were included in the study group and followed up.Through follow-up,the survival time,recurrence rate,postoperative quality of life after the operation were recorded,the data were analyzed using statistical software.The overall survival of the patients who performed partial cystectomy compared with that of the patients who performed radical cystectomy was not statistically significant(p>0.05).The quality of life in bladder preserving treatment patients is better than that of radical cystectomy patients.In two groups of patients,the average number of days of hospitalization was significant difference(p<0.05),The postoperative hospital stay of Bladder sparing treatment group is less than that of radical cystectomy group.Conclusion:In operation refers to the muscle invasive bladder cancer patients,Radical cystectomy for bladder cancer and partial cystectomy combined with chemotherapy of GC regimen can be performed safely and effectively.Among the patients,compared with the other operation treatment,radical cystectomy did not significantly increase the survival rate of the patients.The physical condition of patients,lower pathological stage can obtain higher survival rate after operation.The postoperative hospital stay of Bladder sparing treatment group is less than that of radical cystectomy group.The quality of life in bladder preserving treatment patients is better than that of radical cystectomy patients.The postoperative nursing of bladder sparing treatment group is relatively easy.
