| Purpose: Discussion 3.0T MRI diffusion tensor imaging applications in quantitati ve parameters grade and degree of invasion of preoperative glioma.Materials and Methods: Patients with glioma collected from Beijing People’s Li beration Army Air Force General Hospital in April 2014 to October were treated by surgery and pathology confirmed 21 cases, which according to World Health Organization(WHO) classification method will be divided into three groups of p atients: WHO â…¡grade 5 cases, WHO â…¢grade 9 cases, WHO â…£grade 7 cases, u nderwent routine preoperative MRI scan, T1 enhanced scanning and DTI examina tion. With Functool 4.5 software processing to obtain fractional anisotropy and a pparent diffusion coefficient map FA ADC map and measure FA valuesã€ADC va lue of 4 regions of interest which is the tumor parenchyma, cystic necrosis, perit umoral edema, and normal white matter. Using spss 21.0 statistical software for t he same grade glioma tumor parenchyma and normal white matter(control area),different grade glioma substantive areas, the same grade glioma of different regio n of interest in t test with FA value, ADC values respectively, analysis of wheth er the difference was statistically significant and its correlation with tumor invasi on degree.Results: 1. The significance of FA valuesã€ADC values for evaluating the grade of glioma s. FA values, ADC values of the differences between same-grade gliomas parench yma and normal white matter regions were statistically significant. Tumor essence of ADC values in the WHO â…¡level groupã€the WHO â…¢and WHO â…£level group differences were statistically significant. FA values in the WHO â…¡ level groupã€the WHO â…¢ and WHO â…£level grade differences was statistically signifi cant, but no statistical significance between the WHO â…¢, â…£ level grade. 2. The significance of FA valuesã€ADC values for evaluating gliomas infiltration. In WHOâ…¡ level group, the difference in ADC values between tumor parenchy ma and peritumoral edema, tumor parenchyma and normal white matter, peritumo ral edema and normal white matter were statistically significant; In WHOâ…¢ level group, the difference in ADC values between tumor parenchyma and cystic necro sis, peritumoral edema, normal white matter were statistically significant; In W HOâ…£ level group, the difference in ADC values between tumor parenchyma and normal white matter were statistically significant. FA values in the same grade gliomas turn out by the tumor parenchyma showed an increasing trend, the large st increase occurs in the peritumoral edema and normal white matter.The differen ce in FA values between normal white matter parenchyma and tumor parenchyma,cystic necrosis, peritumoral edema were statistically significant, but The differenc e in FA values between tumor parenchyma, cystic necrosis, peritumoral edema w as no statistical significanceConclusion:1. The quantitative parameters of DTI measurements for evaluating the grade of gliomas has a certain value, FA values were significant differences between high and low grade gliomas, especially ADC values have a certain extent to react th e biological characteristics of glioma, greater value in the hierarchy of preoperati ve prediction glioma. 2. The quantitative parameters of DTI measurements for evaluating the degree of infiltration of glioma has a certain value. The research valuation of ADC values for evaluating the degree of infiltration of glioma is still controversial. FA value can only be quantitative to distinguish tumor area and normal white matter area,can not distinguish between tumor parenchyma, cystic necrosis, peritumoral edem a. Abnormal FA values can quantify the infiltration of the tumor, but not as the criterion standard and can not prove that tumor invasion, because the FA value n ot sure the difference between tumor and edema. 3. The FA can figure with conventional MRI image contrast, show that the degr ee of brain tumor invasion; At the same time can clear stereoscopic display spac e of the tumor and surrounding white matter fiber anatomical position, evaluate t he invaded in white matter fiber tracts, provide important guidance to preoperativ e surgery plan formulation and prognosis. |
