BackgroundEarly abortion is the majority of recurrent spontaneous abortion (RSA), the incidence factors are complex, including embryonic, maternal, fathers factors, environmental and psychological factors in general. Recent studies have shown that genetic factors in men, semen or other factors (such as age and behavioral characteristics) have a significant impact on early abortion. Semen analysis and sperm morphology analysis as important indicators to assess semen quality in clinical investigation of recurrent miscarriage recently, we found that a part of patients with recurrent spontaneous abortion who was ruled out of women’s factors, only based on semen general and sperm morphology analysis didn’t explain the exact cause, only base on semen sperm morphology analysis and interpretation of factors affecting the occurrence of recurrent spontaneous abortion is still controversial, but the sperm chromatin structure analysis from the perspective of genetic material to supplement the male sperm quality roughness methods. In this study, patients with early RSA as the research object, through semen analysis, improved line papanicolaou stain sperm morphology and sperm chromatin analysis of diffusion experiments confirmed that the relationgship of semen quality, morphology, chromatin integrity and early recurrent spontaneous abortion.ObjectiveThe thesis aims to contrast the exclusion of conventional indicators factors semen, the morphological analysis, chromatin damage degree between recurrent miscarriage patients and normal pregnancy after excluding the factors of women’s. So as to analyze the relations among semen quality, morphology and chromatin damage with early recurrent spontaneous abortion. Therefore, the author is trying to provide a new theoretical basis in scientific research and clinical guidelines of early RSA.MethodsThis study selected 90 cases of patients with recurrent spontaneous abortion and 80 cases which had normal reproductive history and infertility. We analyzed all the semen samples. Patients were using masturbation semen and sperm were injected into plastic cups after 3-7 days of abstinence.1.The semen sample was placed in a water bath at 37℃ box waiting for liquefaction. Semen must be assessed within 60 minutes of physical traits (pH value, semen volume), and physical traits. Under the microscope with WLJY9000 type Weili color detection system for sperm quality sperm count, sperm density assessment of sperm motility. Each sperm counted two times the average.2. Modified papanicolaou stain lines sperm morphology analysis, observation mirror 100 times oil smear to observe and assessed vision of each sperm (morphology is not included in the incomplete). Each specimen should be repeated at least 200 sperm observed assessments, statistical normal sperm count, abnormal sperm counts, abnormal head, neck and tail number and middle demorphologyity demorphologyity.3.Sperm chromatin diffusion experiments, counted 200 sperm, sperm observed halo, halo, and accorded to the proportion of the sperm head diameter, divided into large, medium, small and non-Halo 4 levels, no halo and large halo that sperm DNA integrity debris, small and non-halo that sperm DNA fragmentation was fragmentation.Results1.There were no significant differences (P>0.05) between the age of two groups.2.The semen volume, sperm concentration, total activity rate were no significant difference in the control group and study group (P> 0.05), the progressive rate of sperm in study group was higher than the control group, the difference was statistically significant (t=4665,P=0.000)3.there was no significant difference of normal morphology rate, neck and middle and tail demorphologyity demorphologyity rate in the control group and study group (P> 0.05), while the head demorphologyity rate of the study group was higher in the control group, the difference was statistically significant (t=-7.401, P=0.000).4.Sperm chromatin diffusion experiments compared to the situation:large, medium and small halo sperm, no halo sperm and sperm DNA fragmentation rate ratios were statistically significant differences ihe control group and study group (P<0.05).Conclusion1.The progressive rate of earlier abortion of recurrent spontaneous abortion patients’ progressive is lower, spermatozoid head anomalies is higher and the consequences of DNA damage is higher which suggests maybe invovlved in recurrent spontaneous abortion.2.Semen analysis. Sperm morphology could not fully explain the relationship between semen quality in recurrent spontaneous abortion, in order to guide clinical diagnosis it should be further combined with sperm DNA damage detection.
