Objective:Rat ARDS model was established by intravenous injection with oleic acid and Penehyclidine Hydrochloride was applied in the early stage of this model, explore if Penehyclidine Hydrochloride have reduced the degrees of pulmonary edema,relife oxidative stress and whether weaken inflammation have the blow in lung tissue,accordingly research the pathogenesis of ARDS.Methods:1.30 SD rats are assigned randomly to three groups,normal group, model group and treatment group. The rats were anesthetized with chloral hydrate 1.0 g/kg through intraperitoneal injection.dissect of the soft tissue around the femoral vein and femoral artery, put the cannula of self made into the great blood vessel, the Control group was injected of 1.1 ml/kg of saline in.Model group inject of 0.1 ml/kg of oleic acid and 1.0 ml/kg of physiological saline after ARDS model established Interfered group,treated by Penehyclidine Hydrochloride Injection, oleic acid(0.1 ml/kg) was injected via femoral vein and have intravenous injected of 1 mg/kg Penehyclidine Hydrochloride Injection. 2.3 hours later,ampling femoral artery blood from centrifuge extract serum, for measurement of TNF-α and IL-6 levels. 3.The rats were sacrificed and pathological changes observation. The wet weight and dry weight of the lung were measured.Then the rest of pulmonary tissues were taken for histopathological observation and detecting MDA,and the activity of SOD,MPO.Result:1.The index of biochemistry and the left lung index(W/D). compared with control group,the left lung index,the content of TNF-a, IL-6 in serum in model group and the content of MDA in lung tissue, the activity of MPO were significantly increased, the activity of SOD decresed obviously(P<0.05). compared with model group, the left lung index,the content of TNF-a, IL-6 in serum in treatment group and the content of MDA in lung tissue, the activity of MPO were decresed obviously, the activity of SOD significantly increased(P<0.05).2.Pathological changes observed by light microscope.It was shown in model group that the pulmonary interstitial and alveolar edema, alveolar space exudative amoute of protein fluid, neutrophils and red blood cells, Alveolar interstitial broadening obviously. While in the Penehyclidine Hydrochloride Injection interfered group, the changes were much lighter than those in model group,the structure of the control group were generally normal. normal group without this phenomenon.3.Pulmonary ultrastructural changes observed by Transmission electronic microscope:Model group showed evident evacuation of Ⅱ alveolar epithelial cellular lamellar body.while those in treatment group were much lighter than model group. normal group without this phenomenon.4.The content of TNF-α, IL-6 in sera and the activity of MPO, SOD, The content of MDA in lung tissue were correlated to the lung index,Multiple regression analysis showed the change of MDA was the most important factor to lung index.TNF-α, IL-6 in serum and the content of MDA were positive correlation to MPO in lung tissue.and have negative correlation to the acitivty of SOD.Conclusion:1. Acute respiratory distress syndrome in serum TNF-α,IL-6 and MDA,SOD,M PO in lung tissue response degree of lung tissue damage, MDA is the most determinant factor.2.There was positive correlation between the content of TNF-α,IL-6 and MDA can promote the production of MPO,there was negative correlation between SOD in lung tissue,the content of MPO activity in lung tissue,which indicates SOD may decrease the production of MPO or promote deactiviation of MPO.3.Penehyclidine hydrochloride can effective control inflammatory mediator and scavenging oxygen free radicals.having the effect of reduce pulmonary edema and the degree of lung tissue injury was relieved.4.Penehyclidine Hydrochloride has certain therapeutic effect on ARDS.
