Objective:To investigate the value of ST segment elevation of lead aVR in identifying the infarct-related artery and clinical significance in patients with acute myocardial infarction.MethodsWe comparatively analyzed electrocardiogram, clinical angiography, cardiac ultrasound, the peak of cTnI, Killip class and heart incident in236patients with acute myocardial infarction and divided them into elevation group of6lcases and no elevation group of175cases according to the ST segment changes of aVR lead.Results(1) There was significant difference in the patients whose infarct-related artery was left main coronary artery between in the elevation group16cases (26.2%) and no elevation group20cases (11.4%). The patients whose infarct-related artery was proximal left anterior descending coronary artery between in the elevation group34cases (55.7%) and no elevation group56cases (32.0%).(2) AVRSTE left main lesion and proximal left anterior descending branch lesions in patients with STaVR-STV1>0incidence rate difference was statistically significant (78.5%VS8.8%, P<0.05).AVRSTE joint STaVR-STV1>0for left main lesion diagnostic sensitivity/specificity and positive and negative predictive value of30.5%.98.5%,30.5%and98.5%respectively.(3) The cases with the peak of cTnI, Killip class achieve â…¡ and above, and heart incident in elevation group were higher than in no elevation group.ConclusionElectrocardiogram in AMI aVRSTE that left main or left anterior descending branch proximal lesion possibility is big, if STaVR-STVl>0is further support left main lesion. Also indicated that the recent decrease in cardiac systolic function is obvious, infarction myocardial area is larger, the prognosis is poorer, can determine for high-risk patients, should be highly valued.
