A Systematic Review And Meta-Analysis For Promoting Blood Circulation Method In Vascular Dementia Clinical Application

Objective: With the growing pace of aging population in our country and the increasing cerebrovascular desease morbidity and the improving clinical diagnostic level, the onset of vascular dementia is in a trend of fast growth. The TCM literature using promoting blood circulation method in treating vascular dementia has been increasing in recent years. Make a general observation of those TCM literatures, we have observed that many of those researchs have problems such as small simple size、unclear interventions、not randomized grouping design、disunion curative effect evaluation standard and so on. Those problems make the exact efficacy of promoting blood circulation method in treating vascular dementia remains to be further certification. Using the method of evidence-based medicine for promoting blood circulation method in vascular dementia clinical application of literature study system evaluation and Meta-Analysis, so that we can give priority to remove blood method in treating efficacy objectively and credible evaluation, and help to guide the clinical treatment.Method:Using computer to get a comprehensive collection about related literature during January 2000 to December 2014 from large databases above CBM,CNKI, VIP, Wanfang and Medline. Select literature according to the inclusion criteria and exclusion criteria.Then use revised Jadad scale to evaluate the quality of included study. Collected primary data then using Rev Man 5 software to analysis and get credible conclusions.Results: 15 clinical randomized controlled trial literatures comply with t hose criteria. All of them are in low quality and the revision Jadad sco re is 0~2.Through the Meta-Analysis, experiment group and control grou p: 1.Effective rate, P=0.65(P>0.05),I2=0%,no obvious heterogeneity,RR=1.24,95%CI[1.15,1.35],diamond symbol completely on the right of RR=1,which means significant differences were found among experiment gro up and control group. 2.MMSE score, P=0.68(P>0.05),I2=0%,no obvi ous heterogeneity,95%CI[1.82,2.75], diamond symbol completely on the right of WMD=0,that means significant differences were found among e xperiment group and control group. The funnel plot of effective rate sh ows scatters are midline concentrated, but symmetry is poorer.Indicate p ublication bias exist,but bias is small. The funnel plot of MMSE score shows symmetry is poorer,but concentrated on the midline. Indicate publ ication bias exist.The sensitivity analysis of effective rate and MMSE sc ore results suggest literature of high stability and low sensitivity.Conclusion: 1.Promoting blood circulation method treatment of vascular dementia in the effective rate is better than that of western medicine treatment. 2.Promoting blood circulation method treatment of vascular dementia in the MMSE score is better than that of western medicine treatment. 3.Literature have bias,but have high stability and low sensitivity.
