| Objective: To discuss the clinical and histopathologic characteristics and the clinical treatment methods and prognostic factors and the reason of treatment failure inpatient with primary vaginal malignant tumor.Methods: Ten patients with primary vaginal malignant tumor were treated in our hospital between May 1989 and December 2013 were retrospectively analyzed. Patients were staged according to FIGO staging system.One patient had stage â… ; 2 had stage â…¡;4 had stage â…¢ and 3 had stage IV diseases.Five patient had squamous cell carcinoma,2 adenocarcinoma,1 had malignant melanoma,1 had clear cell carcinoma,1 had sarcoma.7 cases were treated by radiotherapy alone,2 cases were treated by radiotherapy in combination with chemotherapy,1 cases was treated by radiotherapy in combination with chemotherapy after operation.Results:The follow-up rate was 90%.4cases were followed-up time over 3 years. 3 cases followed-up over 5 years.The 3-and-5-year overall survival rate were 57% and 42%.Stagewise 5-year survival rate as follow:stage â… 100%, stage â…¡ 50%, stage â…¢ 33.3%,stage IV 0%.5-year survival rate of stage â… -â…¡ were 66% and stage â…¢-â…£ were 20%, the former is significantly higher than the latter.In stage â…¢-â…£ later disease,5-year survival rate of radiotherapy alone and radiotherapy combination chemotherapy were 0% and 50%,respective,there was a significant difference between the two groups(P< 0.05). Local control rate of medium and advanced vagina malignant tumor, in radiotherapy combination chemotherapy group was higher than in radiotherapy alone group,there was a significant difference between the two groups(P< 0.05).Distant metastasis rate of the radiotherapy combination chemotherapy group was lower than radiotherapy alone group,two groups have significant difference (P<0.05).2 cases local control failure and 1 cases distant metastasis all were advanced disease,no one is in the early disease.Conclusion:The prognosis of primary vaginal carcinoma associated with FIGO stage and the clinical treatment methods.Radiotherapy was primary treatment. Conversely, suppression of distant metastasis along with aggressive local control was important for advanced disease,radiotherapy alone was of little value,combination chemotherapy can increase survival rate. |
