Objective : Analysis the Distribution and drug resistance about blood culture in our Hospital from January 2013-December 2014, to guide the use of antibiotics.Methods: I collected 1534 copies of blood culture specimens of our hospital, which result is positive, during January 2013-December 2014. Then use automatic bacteriali dentification and susceptibility analysis system WalkAway96 plus, API verification system, KB disk diffusion method, fungal susceptibility ATB FUNGUS 3kit to identify and make the drug susceptibility testing about the cases. We used the Whonet5.5 software to analyze the result.Results: 1. The distribution of the 1534 copies blood culture isolates: 681 plants of Gram positive coccus(44.39%), most of them are Staphylococcus aureus and coagulase-negative staphylococci; 806 plants of Gram-negative bacilli(52.55%), most of them are Escherichia coli, Klebsiella pneumonia and non-fermentative bacteria; 27 plants of fungi(1.76%). 2. There always some regular of the bacilli disturbution,such as taphylococcusaureus occupied the highest rate in renal department, As well as Enterobacteriaceae always isolate in urology, non-feramentative bacteria always isolate in emergency department / ICU, otherwise enterococcus and streptococcal occupy the most part during cardiac department. 3.The sensitivity of blood culture isolates to antibiotics: during the Gram-positive bacteria, Staphylococcus and Enterococcus has a high resistance to penicillin, erythromycin, and SMZ, but is Sensitive to Vancomycin and Linezolid; The drug resistance of Enterococcus faecium is higher than enterococcus faecalis for most antibiotic which were used to test. At the same time, E.coli is highly sensitive to carbapenems antibiotics; The strains of Escherichiacoli and Klebsiella pneumoniae which producing extended-spectrum β-lactamase(extended-spectrum β-lactamases,ESBLs) respectively accumulated 55.95% and 42.5%. Pseudomonasaeruginosa to cefoperazone/sulbactam, piperacillin/tazobactam, cefepime,meropenem resistance rates≤30.0%. More than 50% of Acinetobacter baumannii is sensitivity to cefoperazone/sulbactam and minocycline.Conclusion: 1.The subsequence of most common bacteria isolate in blood culture is Coagulase-negative staphylococcus, Escherichia coli, Acinetobacter baumannii, Klebsiella pneumoniae, Staphylococcus aureus, Enterococcus, Pseudomonas aeruginosa.2. Though MRSA and MRCNS always found,There no strains was found during the Staphylococcus genera which is resistant to vancomycin and linezolid. During the enterobacteriaceae,.Escherichia coli and Klebsiella pneumoniae have a high rate of product ESBLs, but they are all sensitive to carbapenems. |
