Objective: To investigate the clinical value of endoscopic ultrasonography inpreoperative diagnosis and staging of esophageal cancer.Methods:Collect46cases of esophageal cancer which have diagnosed by routinegastroscopy and pathological, then received the endoscopic ultrasonography beforesurgery and did the preoperative TNM staging, and compared with postoperativepathologic results.Results:(1)The diagnosis accuracy rates were77.1%for T stage with preoperationalEUS of esophageal carcinoma, furthermore the sensitivity, specificity and accuracy ofthe stage T1were60.0%,97.5%,93.4%respectively; the sensitivity, specificity andaccuracy of the stage T2were66.7%,87.5%,84.7%respectively; the sensitivity,specificity and accuracy of the stage T3were78.5%,72.2%,76.1%respectively; thesensitivity, specificity and accuracy of the stage T4were57.1%,94.8%,89.1%respectively. Each T stage of esophageal EUS preoperative and postoperativepathological had no significant statistical difference between groups (T1:P>0.05;T2:P>0.05;T3: P>0.05;T4: P>0.05).(2)The diagnosis accuracy rates were69.6%for N stage with preoperational EUS ofesophageal carcinoma, furthermore the sensitivity, specificity and accuracy of the stageNOwere80.0%,61.5%,69.6%respectively; the sensitivity, specificity and accuracy ofthe stage N1were61.5%,80%,69.6%respectively. Each N stage of esophageal EUSpreoperative and postoperative pathological had no significant statistical difference between groups (NO:P>0.05;N1:P>0.05).(3)The Kappa values for preoperative EUS in T and N stage,as well as postoperativepathological in T and N stage were good (0.4<Kappa value<0.7).Conclusion:The accuracy rate of EUS in T stage were higher for diagnosis inesophageal cancer preoperative staging. EUS has a certain guiding significance foresophageal cancer in developing treatment strategies,and in choosing surgicalapproach,and in estimating prognosis after surgery. |
